This is a full-time 35-hour plus-per-week, non-exempt position. Duties Prepares Youth for Success 
<ul>
<li>Plans and oversees the administration of designated programs and activities that support Youth Development Outcomes.</li>
<li>Establishes program objectives consistent with organizational goals and mission.</li>
<li>Oversees the provision of day-to-day program activities in accordance with established Boys and Girls Club standards and goals.</li>
<li>Ensures that all childcare licensing requirements per the NH Childcare Licensing Bureau are implemented.</li>
<li>Ensures that members are encouraged to participate in a variety of program areas/activities and receive instruction and constructive feedback to develop skills in program area(s).</li>
<li>Demonstrates leadership to ensure proper conduct, safety, and development of members.</li>

 Program Development and Implementation 
<ul>
<li>Establishes and maintains program goals and settings to ensure the health and safety of members. Ensures that program staff understands and effectively communicates standards of the program; that they ensure program areas are safe, well-ventilated, and well-lit; and that Club equipment is maintained in good working condition.</li>
<li>Ensures the evaluation of Club programs on a continual basis, including tracking outcome metrics; verifies that programs/activities respond to member needs and addresses their gender and cultural diversity.</li>
<li>Controls program expenditures within the approved budget.</li>
</ul>
 Supervision 
<ul>
<li>Allocates and monitors work assigned to program volunteers and staff, providing ongoing feedback and regular appraisal. Identifies and supports training and development opportunities for assigned volunteers and staff.</li>
<li>Oversees proper record keeping and weekly reporting, including activities and events conducted, breakdowns of daily participation figures, notable achievements, and any problems/issues.</li>
<li>Ensures productive and effective performance by all program staff and volunteers.</li>
<li>Oversees special programs and/or events (i.e. Keystone, Youth of the Year, and Awards Programs), and/or participates in the implementation of other program activities as necessary.</li>
<li>Increases visibility of Club programs via posting of daily schedules, and announcements of upcoming events through different social media channels, parent emails, etc.</li>
</ul>
 Requirements A program director in a school-age program shall be at least 20 years of age and have a high school diploma or equivalent, including but not limited to a General Equivalency Diploma (GED), a High School Equivalency Test (HiSet), or a Test Assessing Secondary Completion (TASC), and have at least one of the following: (1) Written documentation from or on file with the department that she or he was qualified and employed as a site director in a school-age program on or before the effective date of these rules in 2022; (2) A minimum of an associate’s degree in child development, education, recreation, or other field of study focused on children, awarded by an accredited college or university; (3) Certification of successful completion of training as a recreation director plus 1000 hours of experience working with children in a licensed child care program, recreation program, or a public or private elementary school; (4) A total of 12 credits in child development, education, recreation, or other field of study focused on children, from an accredited college plus 1000 hours of experience working with children; (5) Current certification as an educator by the Department of Education; or (6) Experience working with children totaling 2000 hours and: a. Current certification as a para II educator by the Department of Education; or b. Both of the following: 1. Documentation of enrollment in a course for at least 3 credits in child development, education, recreation, or other fields of study focused on children, through an accredited college or university and a written plan on file for completion of at least 3 additional credits as specified; and 
2. Within 12 months of the date the individual begins working as a site director, documentation of successful completion of a total of at least 6 credits as specified in b.1. shall be on file for review by the department.

Arkos Health is seeking aRN Care Manager. The RN Care Manager will be responsible for working with patients and their families to ensure a smooth transition following discharge from any facility. In addition, the RNCM provides transitional and intensive nurse care management services for the entire network and works collaboratively with the health plan, leadership, department coordinators to ensure the highest quality of patient satisfaction is met.

Essential Duties and Responsibilities:
<ul>
<li>Work directly as a member of the Arkos Health Interdisciplinary Team</li>
<li>Implement and participate in the patients plan of care; update and revise as needed</li>
<li>Assist in coordination of comprehensive care; including obtaining referrals to other interdisciplinary services (Home Health, Palliative Care, Social Services) to help prevent further need for acute care utilization</li>
<li>Maintain case load based on the needs of the client and/or contractual obligation </li>
<li>Conduct face to face visits at a minimum of every 2 weeks for assigned patients enrolled in Intensive Case Management. May conduct increased face to face or telephonic visits based on the needs of the patient and or contractual obligation</li>
<li>Complete Clinical and Social Assessments (CSA) to develop care plan upon initial start of care visit with patient </li>
<li>Provide communication with patients providers and medical specialists regarding patient status and any changes in condition </li>
<li>Engage with Arkos Health leadership and contracted providers to resolve changes in patient status, complaints, or complications though participation in weekly ICT meetings</li>
<li>Complete medical reconciliation for all transitional care patients within 30 days of initiation of TCM contact</li>
<li>For this field-based position, individuals must be able to travel up to 80% of the time in their own personal vehicle</li>
</ul>
 
Qualifications and Skills:
<ul>
<li>Current, unencumbered, Registered Nurse License and graduation from an accredited Registered Nursing program required</li>
<li>Current BLS/CPR certification required</li>
<li>Current TB test and maintain a negative test annually required </li>
<li>Hep B vax/titer proof (Company does offer/or may sign a declination) required</li>
<li>Current flu shot (or sign waiver/follow mask policy) required</li>
<li>Having an in-depth knowledge of health care services and other medical terminology is a required</li>
</ul>

WORK CONDITIONS
Exposure to
Varying and sometimes adverse weather conditions when removing trash and performing other outside tasks. In-store temperatures range from 36 degrees in cooler to 90 degrees and above in some work areas. Sudden changes in temperature in work area and while outside. Fumes from food odors. Exposure to cornmeal dust. Cramped quarters including walk-in cooler. Hot surfaces/tools from oven up to 500 degrees or higher. Sharp edges and moving mechanical parts.
