Case Managers provide education, group facilitation for children and families with children ages 0-5. Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive needs through communication and resources to promote well-being.&nbsp; 
This schedule includes weekday shifts, two evening shifts, and a four-hour shift on Saturdays each week.
Qualifications:
<ol>
<li>Case Manager I - High school diploma or GED required.&nbsp;Bachelor's degree in Counseling, Social Work or related field preferred.</li>
<li>Case Manager II - Bachelor's degree in counseling, social work or related field required. Equivalent experience accepted.</li>
<li>Knowledge of community resources preferred.</li>
<li>Experience in facilitating group sessions, including parent cafes and educational classes for adults and children preferred.</li>
<li>Knowledge of child development framework, including Adverse Childhood experiences (ACEs) and stages questionnaires (ASQ), the Five Protective Factors Survey (PFS) assessment preferred.&nbsp;</li>
<li>Bilingual required.</li>
<li>Must be 21 years old or over.</li>
<li>A valid California driver’s license is required of staff who have regular driving duties, and the driving record must meet the requirements of their position and be approved by the agency’s insurance company.</li>

Qualifications
