 EXPOSURE CATEGORY I
BASIC PURPOSE AND SCOPE:
Under the supervision of the assigned RN Supervisor, and in accordance with the clinic contractual agreement, the Clinic Contract LPN is responsible for, but not limited to, assisting with provision of nursing services as authorized by the participant’s plan of care, physician and supervising RN. The Contract Clinic LPN will comply with agency, PACE and Article 28 clinic regulations and policy and procedures
RESPONSIBILITIES:

<li>Obtains nursing data on the participant including vital signs, heart and lung sounds, bowel sounds and basic bodily systems.</li>

 2. Consistently follows infection control policy and procedures.
 3. Prepares clinical and progress notes, completing documentation and required reports in a timely manner and in accordance with applicable protocols.
 4. Participates in team meetings or other family/participant conferences as may be assigned.

Responsibilities
Matrix Provider – What to Expect:
<ul>
<li>Conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols</li>
<li>Work collaboratively with physicians, case managers, social workers, family members, key caregivers and any ancillary medical personnel as appropriate</li>
<li>Collaborate with Primary Care Physician (PCP) on patient education, provide follow-up</li>
<li>Provide services in a variety of venues to include: Home Visits and Skilled Facility Visits</li>
</ul>
Nurse Practitioner - PRN- Home Risk Assessments
Type: PRN- Flexible-Nurse Practitioner
PAY: Earn from $2,000 up to $10,000 per month (Based on an average of 20-100 completed visits per month, and pay of $100 per completed assessment). Compensation range is based on availability and assessment completion.
Location: Home, SNF, and other community environments in Smethport, Pennsylvania
Hours: Days, Evening, Nights, weekend, Flexible Scheduling
Benefits: PRN providers are non-benefited except for 401(k) and limited Voluntary Benefits 
Mileage Reimbursement plan
 
Qualifications
Must Haves:
<ul>
<li>Master’s Degree required OR commensurate experience and satisfactory completion of NP licensure</li>
<li>Current RN and NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority</li>
<li>Board certified by the AANP, ANCC or the AACCN in a Matrix approved specialty</li>
<li>Have a current BLS, ACLS or CPR certification</li>
<li>Have a Minimum of 1 year experience as a Nurse Practitioner</li>
<li>Have strong computer skills and familiarity with Employee health/medical record software</li>
<li>Possess excellent verbal and written communication skills with patient, clients and colleagues</li>
<li>Be comfortable and flexible with frequent change</li>
</ul>
Travel Requirements:
<ul>
<li>Have the ability to travel, a valid state driver license and able to drive a car, proof of adequate automobile insurance coverage for the state of residence</li>
<li>Travel required to meet patients where they live</li>
</ul>
