FUNCTION:

General Description
ABI Support Professionals provide a range of assistance that is appropriate and necessary for the supported individual’s health and well-being. The ABI Support Professional assists supported individuals to accomplish activities of daily living including but not limited to eating, toileting, dressing, bathing, transferring and ambulation.
ABI Support Professionals report directly to and receive weekly and/or as needed support and supervision from the ABI Home Manager.

<li>#1524 - Sunday-Thursday 3:00pm-11:00pm, 40 hrs/wk</li>
<li>#1531 - Sunday-Thursday 4:00pm-11:00pm, 35 hrs/wk</li>
<li>#1537 - Sunday-Thursday 3:00pm-11:00pm, 40 hrs/wk</li>
<li>#1620 - Sunday, Monday, Saturday 8:00am-4:00PM, 24 hrs/wk</li>
</ul>
Minimum Qualifications
<ul>
<li>High School Diploma or General Equivalency Diploma required</li>
<li>At least 18 years of age</li>
<li>MAP certification preferred</li>
<li>First Aid/CPR certification preferred</li>
<li>Completion of TB screening and testing</li>
<li>Completion of Physical including transportation/transfer addendum</li>
<li>Safe driving record, current driver’s license, personal vehicle, and current motor vehicle liability insurance required, ability to successfully pass criminal background checks</li>

Job Responsibilities
The position includes the following responsibilities. Responsibilities are based upon the needs of the individual supported and may vary depending on their unique needs and goals
<ul>
<li>Communicates effectively with the individual to whom they provide services including his or her family</li>
<li>Provides assistance with activities of daily living including personal care and nutrition</li>
<li>Supports individuals in increasing and/or maintaining their everyday independence within their homes and communities by assisting with light household duties including cooking, laundry and shopping</li>
<li>Provides transportation as needed</li>
<li>Maintains open communication and works with all team members</li>
<li>Completes needed documentation that may include weekly time sheet, daily progress notes, incident reports, medical and dental forms, etc., in a timely manner</li>
<li>Provides reliable and appropriate service to individuals and families</li>
<li>Maintains necessary certifications (CPR, First Aid, Services/Supports, etc.) and completes all required training in a timely manner in accordance with training requirements</li>
<li>Participates in routine established for support and supervision with direct supervisor</li>
<li>Provides supports/services as directed by families who have established supports for their family member</li>
<li>Any other job-related duties as assigned.</li>
