JOB DESCRIPTION: F45 Bothell Trainer
Full Time &amp; Part Time Opportunities Available
We are looking for an ambitious, energetic, motivating, knowledgeable, charismatic Trainers to join our F45 Bothell Team!
As an F45 Trainer you play a vital role within our team, to coach classes, work with an awesome team of personal trainers, and deliver the F45 experience to our community. You will work closely with our leadership team to help current members with their health &amp; fitness goals, and welcome new members thrive.
The ideal candidate is energetic and enthusiastic, with excellent communication skills, is able to overcome objections, and have a willingness to grow and foster a fun and professional membership culture.
This is an onsite position. With majority of time will be spent leading training sessions for all fitness levels from beginner to elite athletes and creating a greater sense of community in the studio that delivers results and high membership satisfaction. Each of our trainers also act as Brand Ambassadors for our Studio, where you may also have opportunities to be paid to attend external events, athletic events, park class sessions for members locally, and networking events to increase brand awareness and drive member retention.
Reports to: Head Trainer
Responsibilities:
· Demonstrate knowledge of the brand and model behavior in accordance with F45 Training policies and standards
· Work closely with the Management team to ensure classes are facilitated at the highest level
· Collaborate with Management team &amp; owners to discuss daily, weekly, and monthly goals
· Assist in studio protocols to remain impeccably maintained
· Ensure that all members are getting attention during class, performing exercises safely and correctly while keeping them motivated and encouraged
· Perform and demonstrate all exercises in class with modifications as needed
· Participate in the safe operation of the studio including compliance with all safety regulations, proper use of equipment and personal protection equipment, timely reporting of incidents of injury, and participation in regular safety training and inspections
· Foster a clean, organized, energetic, and fun studio environment
· Assist the team to perform fitness consultations
· Lead by example in living a healthy lifestyle
· Assist in creating a fun, safe, healthy and community-focused studio culture that delivers high member satisfaction
· Assist in completing fitness appraisals and pre-exercise screening
· Prepare and engage in every class to ensure the safety of your participants in regard to heart rate, exercise technique, posture, and joint stability when performing resistance training
· Maintain working and practical knowledge of physiology and exercise technique to demonstrate, correct, and assist with regressions and progressions of all exercises
Qualifications:
· Preferred 1+ years of Personal Training Experience
· Preferred 1+ years of experience in Group Training preferred
· Required NASM, ACE, NSCS, or other relevant fitness certification
· Preferred Current First Aid and CPR certification

Housing Connector is seeking a Case Manager Support Associate (Contractor) to provide temporary, project-based support to our North Texas community partners. In this role, you’ll assist case managers in effectively using Housing Connector’s online platform, the Community Hub, through hands-on technical assistance, outreach, and feedback collection.
 
You’ll help build confidence among users, identify opportunities for improvement, and share insights with Housing Connector’s Product and Market teams. This contract position is ideal for someone interested in housing equity, technology, and community engagement.

RESPONSIBILITIES

About the Role
Responsible for the operational delivery of the plan’s case management and coordination programs and processes. Provides case management services for CHPW members with short term, long term, stable, unstable, and predictable course of illness, and/or highly complex medical/behavioral and social conditions.
The goal is to improve members' quality of life and ensure cost-effective outcomes by using internal and community-based resources.
The Case Manager level will be determined by the hiring manager based on education, previous experience, and demonstrated leadership skills.
To be successful in this role, you:

<li>Have a Bachelor’s degree in nursing, or a master’s degree in social work and/or related behavior health field (preferred)</li>
<li>Possess Current, unrestricted license in the State of Washington as a registered nurse (RN) (required) OR</li>
<li>Current, unrestricted license in the State of Washington as a Social Worker (LSWAA, LSWAIC) (required), OR</li>

Essential functions and Roles and Responsibilities:
The Case Manager I is responsible for performing telephonic case management for members with acute, chronic, and complex needs.
Examples listed below are not necessarily exhaustive and may be revised by the employer.

<li>Advocates on behalf of members and facilitates coordination of resources required to help members reach optimum functional levels and autonomy within the constraints of their disease conditions.</li>
<li>Works within a multi-functional team to connect with providers, members, caregivers, contracted vendors, community resources, and health plan partners to assess the member's health status, identify care needs and ensure access to appropriate services to achieve positive health outcomes.</li>
<li>Assesses, evaluates, plans, implements, and documents care of members within the organization’s clinical database system, in accordance with organizational policies and procedures.</li>
<li>Responsible for the assessment of members, including identifying and coordinating access to the appropriate level of care and treatment. Uses the assessment information to assign the appropriate risk and complexity level, and create and document a care plan in coordination with the member, family and health team input.</li>
<li>Initiates a plan of care based on member-specific needs, assessment data and the medical/behavioral plan of care. Goals for members are measurable and developed in conjunction with the patient/family to improve quality of life.</li>
<li>Plans care in collaboration with members of the multidisciplinary team, and considers the physical, behavioral, cultural, psychosocial, spiritual, age specific and educational needs of the member in the plan of care.</li>
<li>Reviews and revises the plan of care with the interdisciplinary care team to reflect changing member needs based on evaluation of the members’ status, and/or as a result of reassessment.</li>
<li>Implements the plan of care through direct member care, coordination, and delegation of the activities of the health care team. Promotes continuity of care by accurately and completely communicating to health care team the status of members for whom care is provided. Engages community resources where applicable.</li>
<li>Conducts interdisciplinary care team meetings with the member/family to assess care plan and recommend adjustments as indicated.</li>
<li>Continuously evaluate members’ progress towards goals, identify potential barriers to attaining goals and expected outcomes in collaboration with other health care team members.</li>
<li>Documents all case activity using the CHPW care management system and follows documentation standards and protocols.</li>
<li>Collaborates with the Transition of Care (TOC) team if a member is hospitalized.</li>
<li>Serves as a liaison at various local and statewide meetings and/or workgroups and provides clinical support to providers’ network to enhance integrated care coordination.</li>
<li>Assesses barriers to care and assist members and health care team to address concerns.</li>
<li>Implements developed workflow activities and activities for designated programs.</li>
<li>Conduct member case management in the field at Provider(s) office, member’s home, inpatient medical or psychiatric hospitals, skilled nursing facilities, adult family homes, or in a community setting.</li>
<li>Attend member appointments or care conferences in-person in collaboration with the members care team when indicated.</li>
<li>This position may require traveling on behalf of the Company and working in the field. It is required that a current driver’s license, proof of insurance and an acceptable driving record are maintained.</li>

Case Manager I

Job Description
Law Enforcement Assisted Diversion (LEAD) is a collaborative project involving local police, prosecutors, public defenders, corrections officers and social service providers designed to divert individuals arrested or suspected of low-level property crimes, drug offenses and/or prostitution into social services rather than the criminal justice system. These individuals will be engaged in supportive services with the goal of reducing legal recidivism and improving quality of life.
LEAD case manager respond to police referrals as well as other social contact referrals within the Everett and Lynnwood city limits. LEAD Case Managers will also provide engagement and intensive case management for eligible and willing participants for this diversion program.
Primary responsibilities include providing screening, outreach, long-term engagement and supportive services for participants through intensive case management activities and collaboration with LEAD partners, service providers, housing providers and other community organizations. The case manager will engage participants on the street and at social service provider facilities to establish a working relationship and offer services. Provide advocacy and support for participants within the criminal justice system including court appearances and written communication. Accompany and transport participants to appointments as needed using company vehicles.
Minimum Qualifications
<ul>
<li>Bachelor’s degree in social work, psychology, sociology or other human services-related field required. Undergraduate degree related to social services or behavioral health and two years’ experience working directly with this population or five or more years of relevant experience working directly with this population may be substituted for educational requirement.</li>
<li>Understanding of harm reduction along with a demonstrated passion for serving individuals experiencing homelessness and co-occurring disorders required. Street outreach experience providing services and resources to the populations we serve a plus.</li>
<li>Valid Washington State Driver’s License (local travel on behalf of the Agency is a job requirement).</li>
<li>Available for on-call response rotation to law enforcement for arrest diversion referrals.</li>
