Job Description:
We are seeking an experienced Registered Nurse – Case Manager for a contract position at a healthcare facility in Santa Fe, NM. The ideal candidate will have a strong background in discharge planning, care coordination, and interdisciplinary collaboration.
Key Responsibilities:
<ul>
<li>Coordinate patient care across the continuum, focusing on safe and effective discharge planning</li>
<li>Collaborate with physicians, nursing staff, social workers, and other healthcare professionals to develop individualized care plans</li>
<li>Evaluate patients’ medical, psychosocial, and financial needs to determine post-acute care requirements</li>
<li>Facilitate communication between patients, families, and care teams to ensure understanding of discharge instructions and follow-up care</li>
<li>Identify and address barriers to discharge, including insurance issues, home care needs, and transportation</li>
<li>Document case management activities accurately and in compliance with regulatory standards</li>
<li>Monitor patient progress and adjust care plans as necessary</li>
<li>Ensure continuity of care by coordinating with external agencies, skilled nursing facilities, home health, and other community resources</li>
</ul>
Required Qualifications:
<ul>
<li>License:&nbsp;Registered Nurse (RN) with a valid nursing license in the state of New Mexico.</li>
<li>Experience:
<ul>
<li>At least 3 years of clinical nursing experience.</li>
<li>Minimum 1 year of experience in case management.</li>
<li>Must have experience in discharge planning.</li>
