Job Description
This position provides consultation to clinical staff regarding wound management, urinary continence and ostomy care regarding patients on care with home health and hospice. This person acts as a consultant in a hospital setting in accordance with state licensure requirements, accreditation guidelines, Title 22 regulations, and the organization’s policies and procedures.
Essential Job Functions:
<ul>
<li>Manages Wound Advisor, verifies that each wound type and protocols are appropriate and current. Initiates revisions to plan of treatment as necessary.</li>
<li>Using Wound Advisor, a software program that allows photographs of patients’ wounds to be uploaded to the e-record, is able to provide consultation to clinicians and physicians regarding wound care.</li>
<li>Manages Wound Advisor protocols and makes changes as appropriate.</li>
<li>Mentors staff in use of Wound Advisor.</li>
<li>Performs the following specific procedures when under direct order by the physician:</li>
<li>Application of silver nitrate sticks to cauterize hypergranulation tissue</li>
<li>Scoring of eschar on a pressure ulcer</li>
<li>Sharp debridement of necrotic tissue</li>
<li>Provides services requiring substantial specialized nursing skills</li>
<li>Provides educational classes for the staff with coordination of the Director of Education as indicated.</li>
<li>Provides education and instruction to patients and their families.</li>
<li>Participates and instructs in specialty related in services for the organization and external professional organizations/associates as appropriate/required.</li>
<li>Participates in QI process related to specialty services.</li>
<li>Demonstrates knowledge of OASIS documents, performs OASIS visits as needed on appropriate patients (ostomy or difficult wounds).</li>
<li>Participates as a member of the Infection Control Committee.</li>
<li>Interacts with insurance coordinator and DME providers as appropriate.</li>
<li>Acts as a liaison for the organization with hospitals and other referral sources and provides them with specialty (CWOCN) information and instruction.</li>
<li>In collaboration with Team Coordinators and Case Manager ensures prior authorizations are completed in accordance with policy.</li>
<li>Proactively ensures patients have correct supplies.</li>
<li>Attends and participates in weekly case conferences.</li>
<li>Identifies health and safety risks and implements risk reduction strategies for self and others according to organization’s policies and procedures.</li>
<li>Communicates regularly with supervisor about department issues.</li>
<li>Demonstrates assertiveness, leadership and good interpersonal skills with physicians and clinicians. Maintains good internal and external customer satisfaction.</li>
<li>Monitors case communication follow up notes, e-mail and mailbox and takes appropriate action in a timely fashion.</li>
<li>Accurately and timely records and reports hours worked as required by state regulations.</li>
<li>Demonstrates assertiveness, leadership and good interpersonal skills with physicians and clinicians. Maintains good internal and external customer satisfaction.</li>
<li>Maintains a positive and respectful professional attitude.</li>
<li>Acts as a positive role model within and outside the agency.</li>
<li>Performs other related duties as assigned</li>

Education:

<li>Certificate in Enterostomal Therapy from an accredited school of Enterostomal Therapy or CWOCN Certification Board.</li>

Great group for both new and experienced grads&nbsp; Responsibilities 1.&nbsp; &nbsp; &nbsp; Physical Therapist (PT) administers and analyzes test results to determine client's deficits and provide appropriate recommendations.&nbsp; 2.&nbsp; &nbsp; &nbsp; The PT may examine clients' medical histories, neurological and psychological test results, and information from interdisciplinary specialists in order to determine appropriate therapeutic interventions.&nbsp; 3.&nbsp; &nbsp; &nbsp; The PT provides skilled therapy in an individual session to clients.&nbsp; 4.&nbsp; &nbsp; &nbsp; The PT plans, directs, and conducts habilitative to treat disorders and developmental delays.&nbsp; 5.&nbsp; &nbsp; &nbsp; The PT devises treatment plans, objectives, and goals and determines potential to meet goals for clients. Short and Long Term goals are formulated to achieve functional outcomes as determined by the client assessment. Goals are reviewed periodically, reassessed in therapy and modified, as indicated.&nbsp; 6.&nbsp; &nbsp; &nbsp; The PT interprets findings to clients, their families, interdisciplinary team members, teachers and/or other concerned parties.&nbsp; 7.&nbsp; &nbsp; &nbsp; The PT documents methods applied response to intervention, and progress made by client. The PT maintains appropriate records, participates in interdisciplinary discharge planning, interdisciplinary documentation, which may include the interdisciplinary plan of care.&nbsp; Qualifications

<li>Excellent communication and interpersonal skills</li>
<li>Familiarity with Microsoft Office suite</li>

Job Description:

<li>Coordinates patient care and flow</li>
<li>Escorts patient to exam room, obtains brief history and vital signs and enters data into electronic health record system (EHR). Updates records in EHR for clinician’s review</li>
<li>Assists clinicians with procedures including preparing patients for exam, as appropriate. </li>
<li>Performs requested patient specific tasks for clinicians after the patient has been seen.</li>
<li>Completes all appropriate consent forms, and provides patient education and referral materials as appropriate, to all patients during clinic sessions.</li>
<li>Cleans and stocks exam rooms before clinic, after each patient and at the end of each clinic session.</li>
<li>When performing front desk duties, answers incoming calls and assists with appointment scheduling.</li>
<li>Assists with reviewing the next week’s schedule to ensure necessary supplies are in stock for procedures.</li>
<li>Ensures that e-prescribing tasks and all assigned tasks in EHR are completed in a timely and accurate manner.</li>
<li>Reviews EHR tasks at end of visit, assisting clinicians with any pending tasks in EHR.</li>
<li>Maintains EHR records accurately following SBNC policies and procedures.</li>
<li>Assists in laboratory as necessary.</li>
<li>Completes tasks in case management system on a daily basis.</li>
<li>Enrolls patients in Patient Assistance Program.</li>
</ul>
Pay and Benefits
The pay range for this position is $25.00 - $25.00/hr.
