BAYADA Home Health Care is currently seeking an experienced Speech Language Pathologist, SLP, for a per diem opportunity performing home health visits. 
This position will be providing home health services to adult and geriatric patients in homes throughout Vance, Granville, Franklin, Nash, and Warren counties (all counties required).
Prior home care experience as a Speech Language Pathologist, SLP is a plus, but not required. 
As a home care Speech Language Pathologist, SLP, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
Speech Language Pathologist, SLP Responsibilities:
<ul>
<li>Make home visits to clients in designated geographic territories.</li>
<li>Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients who are experiencing speech, language, communication, or swallowing difficulties.</li>
<li>Develop a treatment plan based on the evaluation, physician’s orders, and recommended goals.</li>
<li>Continually assess and revise the speech therapy care plan, and participate with BAYADA clinicians in the multidisciplinary care plan, as appropriate.</li>
<li>Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.</li>
<li>Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing a state-of-the-art touch pad tablet.</li>
</ul>
Speech Language Pathologist, SLP Qualifications:
<ul>
<li>A current license as a Speech Language Pathologist in North Carolina.</li>
<li>Possess a certificate of clinical competence in speech from the American Speech and Hearing Association, or;</li>
<li>Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification, or;</li>
<li>Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification.</li>
<li>Solid computer skills; prior experience with electronic medical records (EMR) preferred.</li>

In this role, you will work one-on-one with children, implementing individualized behavior intervention plans under the guidance of a Board Certified Behavior Analyst (BCBA). POSITION SUMMARY
The ARK of DC Case Manager provides services and Case Plan Development to consumers and families. Job duties include scheduling staff, tracking the personal finances of resident.
Coordinating the medical appointments of resident, coaching and training employees, writing reports and attending meetings. This is not strictly an administrative position it requires at times working directly with the consumers. It also requires working on a Saturday or Sunday and some evening hours.
The Case Manager Assesses client service needs, capabilities and appropriateness for services; presents options and services based upon a needs assessment. 
The ARK of DC Case Manager also develops an individual service plan of care and services appropriate to the evaluation with the client/family consensus; reviews and updates service plans. Makes referrals and linkages to appropriate agencies for services; coordinates client services and treatment with multiple service providers and agencies.
 
ESSENTIAL FUNCTIONS

<li>Evaluates the quality of services provided and changes in clients condition and counsels clients as necessary; evaluates clients environment for safety, security, negative factors and productivity; compiles and analyzes data relating to complaints; identifies and works to resolve problems.</li>

<li>Plans and facilitates group sessions with clients participating in community activities and/or to promote recovery and educate clients. </li>

<li>The successful applicants will have a strong knowledge base with regard to diagnostic assessment techniques, counseling approaches, and service modalities used for persons with substance use disorders and co-occurring mental illness. </li>

<li>The Case Manager will plan, organize and direct responsibilities of staff to ensure the consumers personal support/habilitation plans are being carried out, always encouraging consumers choice, employment, and independence.</li>

Reviews and monitors the development and implementation of consumer treatment plans and service delivery plans. Attends all licensing reviews and participates in exit interviews.</li>

What We’re Looking For: 
The Sante Group is seeking a Full-Time Substance Use Disorder (SUD) Phone Counselor to join our crisis intervention team in Timonium, MD. As a SUD Phone Counselor, you will provide support to callers across Baltimore City, Baltimore County, Howard County, and Carroll County. These services may include supportive counseling, information and referrals, crisis intervention counseling, suicide prevention and intake screening of callers to the Mobile Teams, In Home Intervention Teams, Urgent Care Center and other services. This also includes dispatching appropriate services when needed as well as providing goal directed counseling, crisis planning, and follow up for clients served. 
What You’ll Do: 
<ul>
<li>Triage incoming crisis calls and obtain client information.</li>
<li>Conduct assessments and dispatch appropriate resources when needed.</li>
<li>Deescalate clients in crisis over the phone.</li>
<li>Develop appropriate and realistic crisis plan and complete appropriate follow tasks.</li>
<li>Effectively communicate clients needs while making internal agency referrals.</li>
<li>Adhere to Lifeline Standards, Crisis Now Standards, and appropriate accrediting bodies best practices when providing services.</li>
<li>Network with local SUD providers to develop and expand resources.</li>
<li>Knowledge and familiarity with community resources, both mental health and non-mental health.</li>
<li>Abide by all federal, state and local confidentiality and reporting regulations</li>
<li>Complete documentation in an accurate and thorough manner.</li>
</ul>
What We Require: 
<ul>
<li>Bachelor’s Degree in Human Services, Social Work, or a related field of study OR ten (10) years of experience working on a crisis hotline.</li>
<li>CAC-AD Certification or eligible to receive the certification</li>
<li>Minimum of five (5) years of experience working in the SUD field.</li>
<li>Knowledge of substance use disorder providers and various levels of care.</li>

The typical pay range for this role is:

Responsibilities for a PT - Physical Therapist include:

<li>Make home visits to clients in designated geographic territories.</li>
<li>Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.</li>
<li>Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents. </li>
<li>Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.</li>
<li>Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.</li>
<li>Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.</li>

Qualifications for a PT - Physical Therapist include:

<li>A current North CarolinaPhysical Therapist - PT license</li>
<li>Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.</li>

<li>Strong interpersonal, organizational, and problem solving skills.</li>
