Shift Structure

<li>Provider documentation is completed in Theoria's proprietary electronic health record (EHR) system, ChartEasy™ </li>
<li>PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems </li>
</ul>
Physical Requirements
<ul>
<li>Must be able to move inside a facility frequently and briskly in order to see patients, meet with managers and facility staff, and attend to patient needs.</li>
<li>Punctual in-person attendance (typically between 7 a.m. and 1 p.m. is required (for rounders).</li>
<li>Continuous operation of app-based EMR/notes system either through a computer or on a mobile device (iPad, Phone etc.).</li>
<li>Ability to ascend and descend stairs in a facility.</li>
<li>Some ability to position oneself to obtain records, tools, medical equipment and other items that may be in locations that are below the knee or above the shoulder.</li>
<li>Effectively communicate with patients, managers, and Facility staff.</li>
<li>Ability to observe details in close range (within a few feet of the observer) in order to provide accurate and appropriate medical recommendations, treatment, and to provide timely updates to Managers regarding facility or patient concerns.</li>

Essential Functions &amp; Responsibilities
<ul>
<li>Operational Management: Oversee day-to-day clinical operations for Theoria-partnered facilities, ensuring efficiency and compliance.</li>
<li>Metrics Monitoring: Track and manage provider timeliness, quality, productivity, and compliance with facility responsibilities.</li>
<li>Quality Initiatives: Collaborate on facility metrics, including Five-Star measures, QAPI initiatives, and reducing rehospitalizations.</li>
<li>Customer Relations Management: Provide superior customer service by addressing partner issues comprehensively and diligently.</li>
<li>Facility Engagement: Conduct regular check-ins with facility partners </li>
<li>Education &amp; Training: Participate in new facility education, training, and retraining as needed.</li>
<li>Technology Utilization: Navigate multiple computer systems, including Monday.com, Slack, Gmail, and Zoom, to manage operations.</li>
<li>Team Collaboration: Work closely with the VP of Clinical Operations and other team members to ensure smooth operations.</li>
<li>Additional Duties: Perform other associated clinical operations tasks as assigned.</li>
</ul>
Qualifications
<ul>
<li>Educational Requirements:</li>
<ul>
<li>Minimum 5 years of leadership experience in Skilled Nursing Facilities (SNF), such as Director of Nursing, administrator, or department director.</li>
<li>Must possess a clinical education credential </li>
</ul>
<li>Skills &amp; Experience:</li>
<ul>
<li>Strong understanding of SNF quality measures, regulations, and expectations.</li>
<li>Experience in employee management and team leadership.</li>
<li>Proficiency in managing work Relative Value Units (wRVU) and productivity metrics.</li>
<li>Tech-savvy with strong computer skills.</li>
<li>Excellent problem-solving skills, including root cause analysis.</li>
<li>Autonomous, independent worker with a strong understanding of healthcare staffing and needs.</li>
</ul>
<li>Physical Requirements:</li>
<ul>
<li>Ability to remain in a stationary position for at least 50% of working hours.</li>
<li>Frequent movement inside facilities to meet with providers and staff.</li>
<li>Ability to obtain records, tools, and medical equipment from various locations.</li>
<li>Effective communication with patients, managers, and facility staff.</li>
</ul>
</ul>
Shift Structure
<ul>
<li>Work Schedule: Monday-Friday, 7 AM-5 PM EST, plus one weekend day per month on-call.</li>
</ul>
Compensation and Benefits

<li>Base Salary: Up to $130,000 annually</li>
