The Supervisor, Intensive Community Care, RN is a multi-market leadership position responsible for leading a team of Intensive Community Managers (ICCMs) in specific markets to deliver on managing their case load by providing onsite and remote planning, clinical group leadership and expert technical guidance to their team. Directly reporting to the Manager of ICC, this Supervisor is responsible for their teams members assigned patient case load whose role is to proactively manage their assigned patients to prevent unnecessary hospital arrivals. The success of this role will be measured by number of hospital arrivals for the patients in their care. The supervisor’s role works closely with the manager to achieve success in assigned markets and provides manager coverage when needed for the region. The Supervisor of Intensive Community Care, RN, Complex Care will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures. The Supervisor of Intensive Community Care (ICC) directly manages specific market teams that will include multiple areas and ICCMs. As a member of the Complex Care Team, this role is responsible for high-value care across multiple regions, to serve as coach, mentor/trainer to all members of care management team, giving guidance in best practices, troubleshooting to optimize the value of care in hospital and community setting.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
<ul>
<li>Supervises a team of Intensive Community Managers (ICCMs) to deliver on reducing the number of hospital arrivals for the patients within their care and for a period of time post program.</li>
<li>Actively involved in the full employment cycle of direct reports, including but not limited to: participation in the recruitment process, annual performance appraisals, day-to-day oversight of staffing/scheduling needs, weekly time, time off approval, team vacation coverage plan, active nursing license verification, problem solving, exit interviews and termination/counseling decisions.</li>
<li>Provides coverage for the Manager of Manager of Intensive Community Care when needed.</li>
<li>Advocates for and assists Intensive Community Managers (ICCMs) with ensuring patients achieve optimal health, access to care and appropriate utilization of resources.</li>
<li>Consults with, encourages and influences physicians, specialists, and other providers to refer patients to care management for assessment, planning, implementation, coordination, monitoring and evaluation to develop an individualized care plan prior to hospitalization which can result in decreased admissions and hospital sick days. Aids in identifying areas of opportunities at the PCP, center, and market level.</li>
<li>Displays leadership skills, mentoring staff to achieve success.</li>
<li>Serves as the resident CM and DM expert to support Intensive Community Managers (ICCMs) needs for market specific education.</li>
<li>Facilitates medication reconciliation and adherence education; disease education and coaching; advance directive/end of life discussions and referral/authorization management with Intensive Community Managers (ICCMs) as needed.</li>
<li>Assesses and reassesses the patient’s progress utilizing telephone and other technologies and resources. Depending on the progress, implements appropriate interventions to obtain optimal outcome.</li>
<li>Educates a team of ICCMs on plans of care as necessary with the healthcare team, family, and providers.</li>
<li>Assists with development and implementation of case management policies, processes, and standard operating procedures (SOPs) as appropriate.</li>
<li>Attends meetings that require Care Management (CM) and Disease Management (DM) participation relay information as needed to all Intensive Community Managers (ICCMs)</li>
<li>Demonstrates how to conduct educational programs for PCP’s, Chen Med staff, and tier one hospital staff.</li>
<li>Identifies and shares best practices to advance efficiency and performance.</li>
<li>Assists with workflow efficiency improvement for clinical and operational practices.</li>
<li>Develops and support the implementation of SOPs</li>
<li>Completes individual plan of care with patients and team members.</li>
<li>Communicates instruction and methodologies as appropriate to ensure that the plan is implemented correctly.</li>
<li>Monitors and manages clinical and financial coordination of treatment plan of assigned patients to ensure timely, cost-effective, individualized service delivery to prevent unnecessary hospital arrivals, ensure patients are on their appropriate medication, and receive appropriate coordinated specialty care as needed.</li>
<li>Facilitates inter-disciplinary rounds in conjunction with market, regional, and home office physicians and leaders for discussion of complex needs and follows up on action items.</li>
<li>Monitors, reports out, and addresses areas of opportunities progress towards goals, KPIs, and daily actions of market teams.</li>
<li>Serves as preceptor when assigned</li>
<li>Monitors the quality, frequency and appropriateness of HHA visits and other outpatient services.</li>
<li>Performs other duties as assigned and modified at manager’s discretion.</li>
</ul>
KNOWLEDGE, SKILLS AND ABILITIES:

EDUCATION AND EXPERIENCE CRITERIA:
<ul>
<li>Associate degree in Nursing required.</li>
<li>Bachelor’s Degree in Nursing or RN with Bachelor’s Degree in related field preferred.</li>
<li>Must be a Registered Nurse with over three (3) years of clinical experience.</li>
<li>Compact license required if available in state.</li>
<li>A minimum of 2 years’ onsite case management at ChenMed and/or utilization review or discharge planning in a hospital at ChenMed.&nbsp;&nbsp;&nbsp; Special permission to hire outside of this requirement may be granted by the Director of CM/DM.</li>
<li>Maintain current professional licensure in nursing or professional field of certification.</li>
<li>Hospital, healthcare setting experience important.</li>

GENERAL SUMMARY
The Patient Service Representative I is responsible for providing customer service and ensuring the patient experience, either by phone or in person, is exemplary. The Patient Service Representative I will coordinate clerical tasks such as answering the phones, greeting patients and visitors, and scheduling appointments in a professional and timely manner. They are responsible for moving the patients through the intake and checkout process including patient registration, scanning and filing medical records, collecting co-payments, deductibles, and any outstanding balances. The Patient Service Representative I must ensure that all procedures, from identifying correct patient files to verifying insurance information, are closely followed to create a seamless patient experience between clerical and clinical staff.
 
ESSENTIAL JOB FUNCTION/COMPETENCIES
Responsibilities include but are not limited to:

<li>Welcomes and greets all patients and visitors, in person or over the phone.</li>
<li>Is responsible for keeping the front desk area clean and organized.</li>
<li>Registers new patients and updates existing patient demographics by collecting detailed patient information including personal and financial information (ex. co-payments and insurance cards).</li>
<li>Collects outstanding patient balances.</li>
<li>Obtains referrals and authorizations when required.</li>
<li>Scans incoming faxes, consents, reports, and all other patient information into patient chart.</li>

<li>Schedules follow up services and office visits for patients.</li>

<li>Keeps medical office supplies adequately stocked by anticipating inventory needs, placing orders, and monitoring office equipment.</li>
<li>Protects patient confidentiality, making sure protected health information (PHI) is secured by not leaving PHI in plain sight and logging off the computer before leaving it unattended.</li>
<li>Ensures proper hand off of responsibilities once their task is completed.</li>
<li>Meets established attendance criteria and starts work promptly. Punctual and dependent for assigned/confirmed shifts.</li>
