Center for Life Management (CLM) provides the area's most comprehensive array of outpatient mental health services for adults and elders. Our Integrated Care Program is an innovative approach to whole healthcare that encompasses the integration of treatment modalities for mental and physical health needs. Working together as a multi-disciplinary team, internal (CLM) and external (Lamprey Health Care) work as one entity to support individuals with comorbid health conditions. 
 
The Integrated Care Case Manager provides care planning, resource identification, benefit coordination, service monitoring, internal and external care coordination, and community-based counseling support to assist clients with improving mental and physical health symptoms and behaviors that otherwise interfere with daily functioning and wellness goals. This is a full time, 35 hour per week position.
 
Major Functional Areas and Description of Duties
<ul>
<li>Provide individual assessment to create care plans that specify presenting problems, objectives, interventions, and desired outcomes.</li>
<li>Assist clients with applying for and maintaining adequate medical coverage to support consistent access to whole health care and treatment.</li>
<li>Provide outreach, advocacy, coordination, resource identification, linkage and monitoring of internal and external supports and services.</li>
<li>Provide counseling and Functional Support Services (FSS) in the home or other community-based setting as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan.</li>
<li>Provide Individual Therapeutic Behavioral Services (TBSI) to develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement.</li>
<li>Provide Crisis Intervention (CI) when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community. </li>
<li>Provide timely documentation on each service delivered in accordance with agency policies.</li>
<li>Work collaboratively with other treatment team members, Healthy Together program, natural supports, and external providers, in particular CFI and their vendors, to ensure a wrap-around approach is provided for continuity of client care.</li>
<li>Provide set case management contacts per month (eligible for incentive pay upon exceeding productivity expectation).</li>
<li>Provide set hours of billable service each week to meet 28.571% of direct service key performance indicator expectation (eligible for incentive pay upon exceeding productivity expectation).</li>
<li>Serve on appropriate committees, both internal and external, as determined by supervisor.</li>
<li>Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients.</li>

Physical Demands and Working Conditions
The majority of the work will be office based and/or remote via telehealth. Some of the work will be visiting clients in the community and also in their homes. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Reliable transportation, a clean driving record, and proof of auto insurance coverage is required.

Essential Functions &amp; Responsibilities

<li>The physician serves as medical director for skilled nursing facilities (SNF) and participates in administrative and quality initiatives for the facilities he or she is responsible for;</li>

Essential Functions &amp; Responsibilities
<ul>
<li>The physician serves as the attending physician for the residents residing at each facility;</li>
<li>The physician collaborates with Nurse Practitioners and Physician Assistants to manage day to day resident care at each facility. Typically, each facility will have a full-time PA or NP assigned to the facility depending on the acuity and volume of the patient population;</li>
<li>Physician will provide high quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs;</li>
<li>Physician is responsible for diagnosing and treating patients with acute illness, such as bacterial and viral infections, as well as chronic diseases and conditions, such as hypertension, heart disease, or diabetes</li>
<li>Provider documentation is completed in Theoria's proprietary electronic health record (EHR) system, ChartEasy™</li>
<li>PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems</li>
<li>Participating in Theoria's Chronic Care Management, Transitional Care, RPM and other medical quality initiatives and programs</li>
<li>Physician must be able to supervise collaborating nurse practitioners and physicians assistants</li>

Shift Structure

The Father's Heart Street Ministry is a nonprofit organization dedicated to serving homeless adults in Oregon City and surrounding areas. 
 
Our mission is to extend hope, help, and resources to those experiencing homelessness, poverty, and crisis by meeting basic needs, as well as providing supportive services and pathways to stability and independence. 
 
Overview
 
The Shelter Staff role provides essential support to participants experiencing houselessness and housing instability. Staff will maintain a safe, clean, and welcoming environment. This role involves monitoring participant activity, supporting daily operations, enforcing shelter policies, and responding to participant needs and crises with compassion and professionalism. This role specializes in connecting the organization’s flexible internal funding and resources with participants who would benefit from emergency financial support. 
 
This is a 24/7 shelter with shifts around the clock. Shifts are typically 10 hours in length. We utilize a rotating schedule of Wednesdays off. The standard days/nights schedule is Sun-Wed or Wed-Sat. This results in an alternating 3-days on and 4-days off, then 4-days on and 3-days off. 
 
Key Responsibilities
 
This is a complex position requiring the ability to adapt to high stress situations and de-escalate conflict. Adherence to HIPAA privacy and general computer skills are required.

<li>Greet and check in shelter participants upon arrival; complete intake and log information accurately. </li>
<li>Determine eligibility requirements for participants who may qualify and benefit from flex funds and rapid-rehousing style support. </li>
<li>Maintain case notes and documentation for expenditure of funds. </li>
<li>Identify participant needs and subsequent appropriate housing and community support. </li>
<li>Research and build relationships with community partners to connect participants to sustainable housing options. </li>
<li>Monitor shelter areas to ensure safety, order, and adherence to shelter rules. </li>
<li>Assist with individual case plans tailored to each participant’s needs. </li>
<li>Provide basic support to participants, including referrals, guidance, and de-escalation when needed. </li>
<li>Respond calmly and effectively to emergencies, including medical or behavioral incidents. </li>
<li>Maintain cleanliness and order in common areas; assist meal distribution, and emergency supply delivery. </li>
<li>Document critical incidents and participant specific data. </li>
<li>Collaborate with case managers, supervisors, and external service providers. </li>
<li>Ensure the shelter environment is trauma-informed, inclusive, and respectful- with particular consideration given to historically marginalized communities. </li>
</ul>
Requirements:
Qualifications
 
Two years’ experience with rapid-rehousing, funding navigation, or housing navigation. High school diploma or GED required; relevant human services or social work experience preferred. Prior experience working in a shelter, residential facility, or with vulnerable populations is a plus CPR/First Aid certification (or willingness to obtain). Background Check
