The role The Nurse Practitioner is an extension of the physician who is responsible for gathering clinical findings to assist with determining continuing eligibility for hospice patients. The Nurse Practitioner is to complete face-to-face visits with hospice patients and coordinating with RN-Case Manager, IDG Team and sponsoring physician on any and all patient needs.
Key Responsibilities:

<li>Performs a face-to-face encounter for Medicare hospice patients prior to the 180th day recertification and every recertification thereafter, and to attest that the encounter occurred under the supervision of a sponsoring physician</li>
<li>Documents clinical findings of face-to-face encounters</li>
<li>Documentation should include:</li>
<li>Evidence of decline in clinical status</li>
<li>Evidence of non-disease specific based guidelines, i.e., PPS, BMI, FAST, dependence in ADL’s</li>
<li>Evidence of co-morbid conditions</li>
<li>Communicates clinical findings to the physician who is considering recertifying the patient</li>
<li>Completes attestation statement regarding date of face-to-face encounter</li>
<li>Submits accurate time and mileage records for all patient related activity to the Medical Director on a bi-weekly basis</li>
<li>Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care</li>

What You'll Do
As a key member of our commercial team, you will:

<li>Build and maintain strong customer relationships as the main point of contact for proposals, pricing, and follow-up</li>
<li>Create, present, and manage proposals using Bid 2024, Excel, and ServiceTitan</li>
<li>Proactive daily coordination with the service team</li>
<li>Partner with management team, dispatch, and technicians to align job timelines, schedules, and project expectations</li>
<li>Order materials, manage purchase orders, and ensure accuracy through coordination with office staff</li>
<li>Review technician "Quote Needed" forms and return pricing promptly to customers</li>
<li>Track open proposals, support preventive maintenance agreement renewals, and assist with billing follow-ups</li>

Schedule &amp; Compensation

Creston is searching for a project manager to serve on its tool management vending team to oversee the installation and maintenance of its tool vending management systems at customer locations. The ideal individual will have strong computer and problem-solving skills with a willingness to learn and implement the vending process and interact with customers directly. Objectives:
Oversees the successful implementation, management, and strategic development and growth of the Company's tool management &amp; vending processes, customer accounts, and programs. Provides guidance and technical support to Creston team members involved in servicing tool managed accounts. Makes sure all customer projects are completed on time and successfully meets customer expectations. 
 
Primary Responsibilities:

<li>Assists in the development of strategies, service, and relationship-building of tool management accounts. </li>
<li>Works with other team members responsible for implementation, service, and modifications. </li>

<li>Monitors and manages accounts to control inventory and ensure adequate margins and profitability.</li>
