What You Will Do:The role of the Patient Care Coordinator is to serve as a liaison between the Clinical staff and the patient to coordinate expenses, clinical care, and administrative needs. The Patient Care Coordinator collaborates with the Nurse Coordinators and takes daily direction from the Nurse Manager.

<li>Assist the RN with new patient appointments and provide support to the nursing staff.</li>
<li>Initiate checklist and chart audit and provide to RN once patient plan is established.</li>
<li>Track and audit all patient consent forms, alert RN of any deficiencies, and conduct patient follow-up for any missing consents or labs.</li>
<li>Track patient workup to ensure current preconceptual labs, communicable testing, pap smear, annual exam, and mammogram (if applicable) are complete.</li>
<li>Send abnormal labs/preconceptual labs to RN for physician review.</li>
<li>Maintain patient charts during ART meeting review.</li>
<li>Educate new patients after initial consultation with physician, prior to discharge, and refer to the primary nurse if immediate cycling is anticipated.</li>
<li>Review and verify all IVF/FET Care plans are completed.</li>
<li>Coordinate treatment plans with RN and physician to monitor testing and treatment cycle and schedule appointments according to treatment protocol.</li>
<li>Release/obtain medical records to/from patients, authorized providers, and insurance carriers; maintain compliance with HIPAA laws and regulations.</li>
<li>Maintain physician correspondence for new patient referrals, graduating patients, clinical notes, embryo transfer follow-ups, pregnancy outcomes, and birth congratulations.</li>
<li>Monitor out-of-town patient lab work and results.</li>
<li>Coordinate with nursing team to ensure lab work/results are recorded in chart.</li>
<li>Monitor IVF treatment process and ensure timely patient flow.</li>
<li>Assist Nurse Manager with situations and patient issues as needed.</li>
<li>Other duties as assigned.</li>

Employed by PAGNY for The Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city.
 
Responsibilities
Scheduler: Monday - Friday, 8am - 4pm 
Under directions of the Director of Operations, with broad latitude for independent judgement and Initiative, schedules staff within the correctional facilities or reassigns staff to ensure that vacancies are Filled. This position is responsible for tracking facility staffing and for relocating staff as needed to meet Demand. In addition, the Staff Scheduler shall be responsible for answering and logging all telephone Calls made to the Operations Office, directing such calls to the appropriate party (s) and paging CHS staff members as needed.

<li>Distributes overtime assignments in an equitable manner.</li>

<li>Maintain patient privacy by adhering to all applicable HIPAA regulations </li>
<li>Promptly address inbound email inquiries and requests from patients and others regarding services and information about OCLI as assigned by manager </li>
<li>Handle making outbound calls to schedule patients upon request </li>
<li>Work collaboratively &amp; courteously with all other departments to ensure a positive patient experience ▪ Perform various administrative functions and other duties/projects as assigned </li>

WHAT YOU'LL NEED TO SUCCEED 
<ul>
<li>Previous customer service experience– Able to manage difficult or emotional customer situations </li>
<li>You are able to focus on solving conflict, not blaming and maintain confidentiality </li>
<li>Must be proficient in relevant computer applications (MS Word, Excel, PowerPoint, Google Suite, etc.) as well as data entry/typing &amp; computerized scheduling systems (EMR/EHR) </li>
<li>Prior inbound/outbound call center and telephone experience preferred </li>
<li>High school diploma or general education degree (GED); or one to two years related experience and/or training; or equivalent combination of education and experience </li>
<li>Experience working with the public &amp; demonstrate strong communication &amp; customer service skills. Previous medical practice experience and/or a general knowledge of current ophthalmology practices, concepts and procedures is preferred. </li>
