The Children’s Home of Reading Youth &amp; Family Services is seeking a Part-Time After School Program Assistant for our Community Prevention Partnership Program.
Purpose:
The primary purpose of the After School Program Assistant is to deliver Kids Against Pressure (KAP) afterschool program to students in elementary schools in the Reading School District. KAP focuses on developing character education, learning conflict resolution, enhancing self-esteem and developing social skills to build positive peer relationships. KAP is delivered through after school sessions, summer KAP camp two-week sessions and approximately 4-6 Saturday enrichment days.
Part-time: approximately 10-15 hours a week during the school year, variable during Summer
Qualifications:
<ul>
<li>Associate's degree in human services, education, social work, health, psychology or a related field is recommended.</li>
<li>Strong communication and interpersonal skills (e.g., nonjudgmental, objective, reflective, empathic, patient, tactful)</li>

<li>Administer pre and post-tests</li>
<li>Assist KAP Program Manager in outreach, recruitment and enrollment of eligible students</li>
<li>Assist KAP Program Manager in maintaining accurate attendance records</li>
<li>Assist in ordering food and preparing snacks for students</li>

ESSENTIAL FUNCTIONS

<li>Utilize classroom and lab instruction, student assessment mechanisms, and classroom management techniques to provide an engaged classroom experience to students.</li>
<li>Ensure that each student has been presented with the material to successfully meet the learning objectives of each course within a student’s program of study</li>
<li>Ensure that instructional materials and equipment are ready for student use, sufficient in quantity, are properly maintained and are in working order prior to class time</li>

MINIMUM REQUIREMENTS
<ul>
<li>High school diploma, GED, and/or proof of completion of related vocational training program.</li>
<li>Verifiable work experience including a minimum of five years verifiable Dental Assisting experience within the last ten years</li>
<li>Must be certified or licensed where required by law</li>
<li>Strong written, oral, and interpersonal communication skills with demonstrated success in collaborating with diverse groups of employees and customers</li>
<li>Demonstration of highest levels of integrity and professionalism in all aspects, including appearance, demeanor, and attitude</li>

What your day looks like:
<ul>
<li>Assist lead teacher in the development and implementation of lesson plans based on developmentally age-appropriate activities, to provide the children with a consistent daily routine. </li>
<li>Assist in the implementation of lesson plans for activities based on the needs of the children and prepare a suitable physical and nurturing environment for ALL children so that they may grow physically, emotionally, and intellectually. </li>
<li>Ensure that the Child Enrichment Center meets licensing and all other legal compliance regulations including Department of Human Services (DHS), Keystone STARS, Pre-K Counts, and Child &amp; Adult Care Food Program (CACFP). </li>
<li>Maintain a team atmosphere within the room. </li>
<li>Assist in the instruction and implementation of daily activities and creation of a positive, stimulating room environment. </li>
<li>Assess the children in their classroom by using developmental assessments twice per year. Assists with recording data into the Teaching Strategies Gold System before scheduled deadlines. </li>

<li>Participate within the guidelines of the Pre-K Counts grant, by meeting the requirements of the mentoring expectation within the partnership agreement. </li>
<li>Stay familiar with all YWCA Lancaster policies and procedures, Childcare Manual procedures, Keystone STARS to ensure program quality </li>
<li>Orient new parents and staff to room and meet with parents when needed. </li>
<li>Attending monthly staff meetings. </li>
<li>Attend a minimum of 27 hours of training per year. </li>
<li>Maintain a current Professional Development Plan for Keystone STARS. </li>
<li>Advocate for Early Childhood Education programs in Pennsylvania. </li>
<li>Enforce center policies with parents, students, and children by upholding and demonstrating the YWCA mission. </li>
<li>Perform other duties as assigned and/or required by Pre-K Counts grant guidelines. </li>

Who you are:

<li>A bachelor’s degree in early childhood education from an accredited college or university in early childhood education and two years’ experience related to the care and development of children. </li>
<li>Certification in early childhood education preferred. </li>
<li>Must be able to lift 45 lbs. and have movement ability to sit, stand, crawl, climb, lift overhead, bend and move from sitting position on floor to standing position. </li>

ESSENTIAL FUNCTIONS:
Receives and responds to all incoming hospice referrals.
Creates the initial referral record in the electronic health record and starts intake process through virtual intake platform.
Schedules admission visits with patient families with goal of 4-hour response
Retrieves and uploads pertinent medical records from hospital electronic health record platforms.
Monitors open/pending referrals and conducts necessary and appropriate follow up
Verifies patient benefit information and obtains initial prior authorizations as necessary.
Answers calls from patients, families, and referral sources regarding hospice services.
Collaborates with supervisors/leadership regarding high risk and complex referrals.
Consistently conducts daily job responsibilities in a manner that maintains excellent customer service for patients, families, referral sources and staff.
Recognizes that quality improvement is a part of every staff member's job responsibility.
Maintain confidentiality of patient information in accordance with HIPAA regulations.
QUALIFICATIONS:
High school diploma or GED required. 
A minimum of three years' experience in customer service or intake in a healthcare setting
Excellent communication skills (verbal and written)
Ability to multitask and prioritize in a fast-paced environment with a high level of accuracy.
Ability to work independently; self-motivated
Proficient at basic phone and computer skills
