Key Responsibilities
Physician Assistants provide primary and/or consultative professional care in a variety of care settings. The Physician Assistant will function in collaboration with physicians according to a Collaboration Arrangement. The Physician Assistant practice will focus on caring for geriatric and seriously ill populations to assist patients and families with comfort and quality of life. 
In this role you will be responsible for:
<ul>
<li>Secure patient signatures on necessary documents and properly document in EMR when necessary at a visit. Review HIPAA Notice of Privacy, consents for treatment, release of records and patient responsibilities forms with patients/caretakers.</li>
</ul>

<li>Drives between locations, navigating to each location using GPS device or other tools if required. </li>
</ul>
<ul>
<li>Assess health status by obtaining relevant health and medical history. Performs a physical exam based upon the age and patient history as per the Collaborative Agreement with a collaborating physician in the patient's home environment. Identifies significant health and medical risk factors. </li>
</ul>
<ul>
<li>Performs Annual Wellness Visits and other quality driven assessments to provide patient with preventative services and understanding of disease progression. </li>
</ul>
<ul>
<li>Orders, interprets and evaluates laboratory tests and other diagnostic studies to assess patient's clinical problems and health care needs. Develops a diagnosis by analyzing and synthesizing collected data. </li>
</ul>
<ul>
<li>Performs diagnostic or therapeutic interventions consistent with certification, law and state medical privileges. May prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board Agency </li>
</ul>
<ul>
<li>Develops a treatment plan in collaboration with the patient and significant others. Participates in medical team rounds; collaborates with nursing, medical and other healthcare team members to ensure quality patient care. </li>
</ul>

<li>Refers patients to physicians as appropriate for consultation or for specialized health resources and treatment. Ensures continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas as necessary. </li>

<li>Provides patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits.  </li>

Required Qualifications

<li>Master's or doctoral degree as a Board-Certified Physician Assistant </li>

Responsibilities for a PT - Physical Therapist include:

<li>Make home visits to clients in designated geographic territories.</li>
<li>Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.</li>
<li>Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents. </li>
<li>Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.</li>
<li>Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.</li>
<li>Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.</li>

Qualifications for a PT - Physical Therapist include:

Essential Functions &amp; Responsibilities
<ul>
<li>The physician serves as medical director for skilled nursing facilities (SNF) and participates in administrative and quality initiatives for the facilities he or she is responsible for;</li>
<li>Medical directorship functions include attending and participating in monthly quality assurance and process improvement meetings; participating in quality improvement initiatives; providing guidance to facility staff; reviewing and revising (if necessary) facility's clinical guidelines; insuring compliance with state and federal regulations, training facility staff; and supervising facility clinical staff.</li>
<li>In addition to the medical director role, the physician also serves as the attending physician for the residents residing at each facility;</li>
<li>The physician collaborates with Nurse Practitioners and Physician Assistants to manage day to day resident care at each facility. Typically, each facility will have a full-time PA or NP assigned to the facility depending on the acuity and volume of the patient population;</li>
<li>Physician will provide high quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs;</li>
<li>Physician is responsible for diagnosing and treating patients with acute illness, such as bacterial and viral infections, as well as chronic diseases and conditions, such as hypertension, heart disease, or diabetes</li>
<li>Provider documentation is completed in Theoria's proprietary electronic health record (EHR) system, ChartEasy™</li>
<li>PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems</li>
<li>Participating in Theoria's Chronic Care Management, Transitional Care, RPM and other medical quality initiatives and programs</li>

Shift Structure

<li>Shifts are flexible depending on physicians' schedule;</li>
<li>Typically, medical directors work a minimum of 2-3 days/week at each facility they oversee;</li>
<li>Rounding times are flexible with no set start and end time. Preferably, physicians start in the morning between 7-10 a.m. and finish their day when their caseload has been seen;</li>
<li>No nights, or on-call</li>
<li>Call is covered by Theoria's virtual care team</li>
