Collaborates with medical staff to develop patient specific medication treatment plans resulting in safe and effective use of medications, conducts and documents patient medication use history specifically for patient transition of care activities from emergency services, inpatient admission to discharge and the next healthcare provider. This position is safety sensitive and FLSA non-exempt. The pharmacist will also be provided with opportunities that include:

Hours/Shift: Full Time/Rotating Key Responsibilities: Serve as pharmacy resource for providing timely and accurate drug information patients, medical staff, nursing staff and ancillary department personnel. 
<ul>
<li> Coordinate and participate in pharmacy services including procuring, storing, preparation, dispensing, administration, and monitoring drugs along with other pharmaceutical supplies.</li>
<li> Develops evidence based and cost-efficient medication treatment plans in collaboration with physician.</li>
<li> Provides timely and accurate pharmacy education to patients, and medical staff including students and residents.</li>
<li> Ensures adherence to Department of Pharmacy regulatory and fiscal requirements.</li>
<li> Coordinates medication management activities related to patient transitions of care from hospital admission to discharge with Pharmacy, Emergency Department and other hospital staff.</li>
<li> Performs or supervises pharmacy staff performing tasks to obtain accurate medication history.</li>
<li> Assists in generating quantitative data on the effectiveness of medication reconciliation process for analysis of regulatory compliance, clinical and economic outcomes.</li>
<li> Coordinate activities to provide discharge medication prior to patient discharge from the hospital.</li>
<li> Participates and assists in clinical pharmacy activities, including but not limited to Therapeutic Drug Monitoring, parenteral nutrition management and medical oncology.</li>
<li> Serves as pharmacy mentor/leader to pharmacists, pharmacy residents, pharmacy students, and technicians.</li>
<li> Assists in quality improvement process, and continued evaluation and improvement of our patient transition of care activities.</li>
<li> Participate and/or coordinate the preparation, labeling and dispensing of drugs and other pharmaceutical supplies; ensure regulatory compliance.</li>
<li> Compound, package and label ointments, solutions and bulk pharmaceutical supplies according to standard formulas; mix prescribed drugs with intravenous solutions as required. </li>

Qualifications

<li>Education: Graduate of an ACPE-accredited School of Pharmacy with PharmD or equivalent training</li>
<li>Licensure: Currently licensed in the State of Arizona preferred, will consider candidate eligible for AZ Licensure</li>
<li>Other: ASHP accredited Pharmacy Residence Program or Board of Pharmacy Specialties Certification (BPS) </li>
