Essential Functions &amp; Responsibilities

Shift Structure

Position Summary 
The Clinic LPN is responsible for providing direct and indirect patient care in the clinic setting. Provides care that reflects initiative, flexibility, and responsibility indicative of professional expectation with minimal supervision. Determines priorities of care based on physical and psychosocial needs, as well as factors influencing patient flow through the system. Communicates with outpatient clinic physicians about changes in patient's status, symptomatology and results of diagnostic studies. Responds quickly and accurately to changes in condition or response to treatment. 
General Duties, Tasks and Responsibilities 
<ul>
<li>Performs accurate and complete assessment of patients presented to the Clinic; records chief complaint; assesses and reassesses pain; assessments are age, culture and psychosocial appropriate. </li>
<li>Adheres to scheduling template guidelines when scheduling new and return appointments; keeps schedules current; documents no show, rescheduled and/or cancelled appointments; creates telephone encounters as needed or directs calls to the appropriate staff for triage; schedules and follows-up on referral appointments and/or testing/procedures.</li>
<li>Educates the patient/ family regarding treatment plan and follow up instructions given per provider instructions including any education handouts. </li>
<li>Assists physician(s)/providers with procedures; utilizes other departments to provide interdisciplinary care as appropriate; utilizes pain management techniques. </li>
<li>Follows medical records documentation guidelines regarding timeliness of reviews and completeness of documentation; obtains and documents accurate and thorough patient medical history including pertinent history, pain assessment, current medications, identified health risk factors and other needed information. </li>
<li>Prepares and administers medications based on age / clinical condition of the patient per policy and nursing standards of care; manages supply of medications and contacts pharmacy as needed to ensure uninterrupted medication therapy. </li>
<li>Receives patient phone calls and appropriately triages the patient condition and assists the patient as appropriate. </li>
<li>Completes telephone encounters as needed and directs calls to appropriate staff; schedules referral appointments, testing and procedures. </li>
<li>Completes pre-certification for testing/procedures; ensures notification of test results for each patient is completed in a timely manner. </li>

The Convenient Care Nurse/CMA is responsible for assessing the needs of each patient: infant, pediatric, adolescent, adult and geriatric. Plans the care needed to meet those needs, including the spiritual, emotional, and physiological needs. Once planned, the RN is responsible to see that the plan of care is carried out through performance and/or leadership of subordinate personnel. Administers IM and oral medications. Responsible for assessment and discharge of patient along with patient and family teaching.

About the Position: 
During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

Responsibilities
Matrix Provider – What to Expect:
<ul>
<li>Conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols</li>
<li>Work collaboratively with physicians, case managers, social workers, family members, key caregivers and any ancillary medical personnel as appropriate</li>
<li>Collaborate with Primary Care Physician (PCP) on patient education, provide follow-up</li>
<li>Provide services in a variety of venues to include: Home Visits and Skilled Facility Visits</li>
</ul>
Nurse Practitioner - PRN - Home Risk Assessments
Type: PRN - Flexible-Nurse Practitioner
PAY: Earn from $1,800 up to $9,000 per month (Based on an average of 20-100 completed visits per month, and pay of $90 per completed assessment). Compensation range is based on availability and assessment completion.
Location: Home, SNF and other community environments in Sullivan, Indiana
Hours: Days, Evening, Nights, weekend, Flexible Scheduling
Benefits: PRN providers are non-benefited except for 401(k) and limited Voluntary Benefits 
Mileage Reimbursement plan
 
Qualifications
Must Haves:
<ul>
<li>Master’s Degree required OR commensurate experience and satisfactory completion of NP licensure</li>
<li>Current RN and NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority</li>
<li>Board certified by the AANP, ANCC or the AACCN in a Matrix approved specialty</li>
<li>Have a current BLS, ACLS or CPR certification</li>
<li>Have a Minimum of 1 year experience as a Nurse Practitioner</li>
<li>Have strong computer skills and familiarity with Employee health/medical record software</li>
<li>Possess excellent verbal and written communication skills with patient, clients and colleagues</li>
<li>Be comfortable and flexible with frequent change</li>
</ul>
Travel Requirements:
<ul>
<li>Have the ability to travel, a valid state driver license and able to drive a car, proof of adequate automobile insurance coverage for the state of residence</li>
<li>Travel required to meet patients where they live</li>
</ul>
