Soldiers on active duty enroll in the program when they have a Family member with special medical and/or education needs requiring specialized services. EFMP ensures that a family member's needs are considered during the military personnel assignment process so their needs can be considered during the military personnel assignment process.
 
Systems Navigators and ACS Compliance Specialists (SNACS) - Exceptional Family Member Program (EFMP) - Systems Navigator (SN)
 
System Navigators are responsible for daily coordination to ensure the facilitation of networking between Families who have special needs and the systems of required care. Duties of the position include intake interviews and documentation, identification needs assessment of the Family member, referrals, follow-up care and information, advocacy, and collaboration with providers in both the civilian and military community.
 
Job Duties

<li>Provide home visits for the Soldier and or Family members to offer support as needed and/or requested.</li>
<li>Coordinate with Government agencies and civilian service providers to identify the availability of non-EFMP respite care resources.</li>

Responsibilities
What you will do: Responsibilities listed include but not limited to:
<ul>
<li>Perform evaluations and develop treatment plans</li>
<li>Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences</li>
<li>Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources </li>
<li>Communicate patient’s needs and progress to the treatment team, physician, person receiving services and family members</li>
<li>Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes</li>
</ul>
 
Qualifications
What you will need:
<ul>
<li>Minimum of bachelor’s degree in Physical Therapy from a college or university with accredited Physical Therapy program </li>
<li>Current, unrestricted license as Physical Therapist by state in which practicing</li>
<li>Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance state regulations</li>
<li>A minimum of one year’s work experience as a physical therapist preferred</li>
<li>Demonstrates knowledge of rehabilitation techniques related to complex neurological injury </li>
<li>Communicates effectively and professionally in verbal and written interactions </li>
<li>Ability to lift 50 pounds</li>
<li>Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures</li>
<li>Duties require fine motor skills,visual acuity, and walking/ standing for extended periods</li>
<li>Additional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times</li>
<li>A health screen or examination may be required prior to assignment and periodically thereafter, depending on the specific location policy, local or state regulations to verify that he/she is physically capable of performing assigned duties with or without reasonable accommodations</li>
