Summary:
Under the supervision of Registered Nurses, Assistant Medical Director, and Treatment Director this individual will intake, monitor, and medicate individuals with acute withdrawal symptoms. You will work as a part of inter-disciplinary team providing support for patients as the transition within Detox &amp; Withdrawal Management as well as participate in discharging patients out of the program or to other levels of care.&nbsp;
Essential Duties and Responsibilities:

<li>Assess, plan, implement, monitor, and evaluate medical and nursing care of all patients.</li>
<li>Explain procedures and treatments to patients to gain cooperation understanding and to alleviate apprehension.</li>
<li>Refer patients to other team members when needs are beyond the scope of practical nursing.</li>
<li>Order necessary medical supplies.</li>
<li>Administer prescribed medications and treatments in accordance with Assistant Medical Director instruction and approved nursing standards and recording dosages.</li>
<li>Coordinate referrals for patients and follow up on medical referrals.</li>
<li>Respond to life-threating situations based on nursing standards, policies, procedures, and protocols.</li>
<li>Provide health education to patients.</li>
<li>Document clearly and completely services provided, patient response and any changes noted.</li>
<li>Document patient history and biopsychosocial status. Document care provided and maintain patient medical records; complete and ensure required forms are completed accurately and completely.</li>
<li>Maintain knowledge of and compliance of applicable federal and state regulations and laws, as well as policies and procedures regarding medication dispensing and narcotic treatment.</li>
<li>Assist the&nbsp;with history and physicals and intakes as required.</li>
<li>Receive medical orders and ensure they are documented and understood by other staff.</li>
<li>Maintain absolute control and tracking of all medical paperwork involving patient does changes, lab results, medical provider orders, phase/level changes, and verbal orders.</li>
<li>Participate in regular nursing meetings and trainings.</li>
</ul>
Required Experience:
<ul>
<li>This individual must have a certificate in nursing from a college or technical school.</li>
<li>Must have a current License for Practical Nursing certified by the MN Board of Nursing</li>
<li>Must have training in co-occurring disorders.</li>
<li>Must be CPR and First Aid certified or obtain it before direct patient contact.</li>
<li>Must pass background studies issued by the Minnesota Department of Human Services.</li>

The Center for Alcohol &amp; Drug Treatment is seeking an experienced Licensed Alcohol and Drug Counselor (LADC) or an Alcohol and Drug Counselor with Temporary Permit (ADC-T) to join our team. In this role, you will play a key part in supporting patients on their journey to recovery by providing direction, guidance, and therapeutic support within our Clearpath program. Clearpath is a non-profit Opioid Treatment Program (OTP) using Medications for Opioid Use Disorders (MOUDs) alongside counseling services to treat patient Opioid Use Disorders.&nbsp; Summary The Center is looking for compassionate LADC/ADC-T to help provide direction and support to our patients. You will actively help patients build coping mechanisms to guide them through stressful situations. Your duties will also include listening to patients, asking necessary questions, and developing effective strategies to improve their lives. As a Counselor within Clearpath you will work with patients specifically with an opioid substance abuse. You'll manage of a case load up to 60 patients and meet with them once a month at the minimum. The CORE FUNCTIONS of this position are as follows: screening, intake, orientation, assessment, treatment planning, counseling (group, individual and family), case management, crisis intervention, client education, referral, reports, record keeping, and consultation with other professionals Programs will include SUD and Mental health services Responsibilities:

<li>Creating effective treatment plans that include counseling, case management or other services within the 12 core functions.</li>
