Summary/Objective: 
Primary job responsibilities involve assessing needs, service and resource linkage, and care coordination to support youth and families/ caregivers in the OhioRISE plan in achieving their health and outcomes goals. Care Coordinators are primarily community-based and may be able to perform some tasks from home. Care Coordinator Non-Licensed will work with individuals, parents/guardians, Boundless staff, community members, and other service and support providers via face-to-face engagement, telephone, video conferencing, and electronic communication. Day-to-day activities are varied based on the needs of the individuals and families/ caregivers on the caseload. The Care Coordinator Non-Licensed serves as a primary point of contact and liaison for all the vital support providers in a youth or family's life, including scheduling meetings, tracking, exchanging documentation, following up on needs and appointments, and reporting outcomes.

<li>For youth and children enrolled in the OhioRISE plan, facilitate the transition, as appropriate, out of the OhioRISE program.</li>
<li>Provide on-call availability and support for youth and families/ caregivers outside regular business hours, as needed.</li>
<li>Identify formal and informal resources in the communities served, initially and on an ongoing basis, paying particular attention to the availability of culturally responsive resources. Link individuals and families served to the resources as needed and update databases to share resource information.</li>
<li>Track and report when less than two providers of a needed service are available within a community to offer an individual or family. Participate in team and community meetings regarding necessary resource development specific to the communities served.</li>
<li>Facilitate the completion of necessary forms and documentation, including consent for release information with relevant stakeholders and child and family teams.</li>
<li>Establish and maintain positive relationships with the local system of care partners such as the children's services system, Title IV-E agencies, boards of developmental disabilities, local corrections and court systems, schools, family and children first councils, managed care organizations, providers of behavioral health services, and primary care providers.</li>
<li>Support individuals and families in accessing medically necessary services and supports.</li>
<li>Serve as a liaison between individuals, families, and care team members to positively promote collaboration.</li>
<li>Complete all documentation as required, including gathering and reporting of all health and outcomes data.</li>
<li>Manage phone calls and communication from individuals, families, guardians, and stakeholders, etc. Coordinate meetings and ensure information is exchanged consistently.</li>
<li>Respond to and be available for clients and families in crisis, as needed, including referencing the crisis plans and facilitating the appropriate linkage to de-escalate the crisis.</li>
<li>Meet engagement, documentation, and contract requirements as established by management.</li>
<li>Provide excellent customer service and communication coordination between team members, individuals, families/ caregivers, community members, and collaborating organizations.</li>

Education/Experience:

As a Software Engineering Manager, you will be responsible for leading, growing, and developing a team of innovative Software Engineers in designing, implementing, and delivering high-quality software products. To be successful in this role, you must be able to lead effectively in a fluid, fast-paced agile environment. This position requires strong technical, design, communication, and collaboration skills. We have aggressive goals over the next couple of years and are looking for candidates with a passion for solving large scale problems and a can-do attitude! Your role: 
<ul>
<li>Provide leadership and oversight to the Software Engineering group. </li>
<li>Oversee team efforts to apply the principles of secure software engineering to the design, development, maintenance, testing and evaluation of software and cloud infrastructure. </li>
<li>Engage with cross-functional leadership to understand business requirements, architecture and technology direction and evaluate alternative approaches, ensuring software and systems are designed accordingly.</li>
<li>Incorporate state-of-the-art concepts and practices to facilitate system functionality, scalability, resilience and security, as well as ease-of-use (developer / user experience).</li>
<li>Ensure appropriate application of agile principles to software development with a focus on delivery of business value, collaboration and continuous improvement. </li>
<li>Oversees Software Engineering processes and teams.</li>
<li>Provides direction and oversight in the planning and delivery of product / solution releases. </li>
<li>Ensures delivery of secure, high quality, well tested and scalable software to meet requirements of the customer and business.</li>
<li>Assists with maintenance and prioritization of the product backlog.</li>
<li>Engages with customers, end users and business partners to understand issues with software development and deployment.</li>
<li>Ensures issues are resolved in a timely and sustainable fashion.</li>
<li>Advocates secure coding practices and testing requirements.</li>
<li>Must be able to influence other software and IT managers to design and build solutions that meet our needs.</li>
<li>Manage career progression for team members, including hiring, performance reviews, salary decisions, mentoring, coaching, and all duties normally associated with a hiring manager.</li>
<li>Represent your team in strategy and planning meetings.</li>
<li>Work with your team to plan, design, implement and operationalize innovations and improvements to existing programs.</li>
<li>Work with your team to continually improve their processes and develop best practices for agile development that delivers value to our stakeholders.</li>
<li>Respond quickly and effectively to production issues and taking responsibility for seeing those issues through resolution.</li>
<li>Stay current with changes in the technical area of expertise.</li>
<li>Share knowledge by effectively documenting plans, decisions, and task ownership.</li>

 Qualifications: 
<ul>
<li>Demonstrate experience executing Agile methods for software product development. </li>
<li>Looking for developers with hands-on experience in C# or similar programming languages and a willingness to adapt to new technologies.</li>
<li>Very strong business analysis, system analysis, or product analysis skills.</li>
<li>Ability to work well in a team-oriented environment.</li>
<li>Excellent planning and prioritization skills with the ability to multitask and adapt to change.</li>

<li>Bachelor's degree in a related discipline and 6 years' experience in a related field. The right candidate could also have a different combination, such as a master's degree and 4 years' experience; a Ph.D. and 1 year experience in a related field; or 18 years' experience in a related field.</li>

 USD 119,600.00 - 199,400.00 per year Compensation: Compensation includes a base salary of $119,600.00 - $199,400.00. The base salary may vary within the anticipated base pay range based on factors such as the ultimate location of the position and the selected candidate's knowledge, skills, and abilities. Position may be eligible for additional compensation that may include an incentive program.
