Responsibilities/duties:

<li>Coordinate and attend appointments with providers such as doctors, therapists, diagnostic testing, transportation, home care services, DME needs, OT and home safety evaluations.</li>
<li>Secure durable medical equipment.</li>
<li>Contact and coordinate medications with Pharmacy.</li>
<li>Research specialty programs for treatment of brain injury, pain management, spinal cord and or other medical issues.</li>
<li>Engage patients and their families, guardians and caregivers as part of the care team to advocacy, ongoing communication, service facilitation an identification of resources.</li>
<li>Advocate, educate, and coordinate care with interdisciplinary team members to maximize client objectives.</li>
<li>Must be able to adapt to using a software system for tracking and documentation purposes.</li>
<li>Request and obtained medical records and insurance resources.</li>
<li>Identify treatment options and community resources.</li>
<li>Communicate verbally and/or in writing with insurance carriers, attorneys to explain the need for services/advocacy of care.</li>
<li>Understand, identify, and coordinate care accordingly as it relates to coordinated insurance policies.</li>

<li>Provider is responsible for timely documentation of all patient care/encounters </li>
<li>The provider will be available for phone/secure messaging consultations with facility nursing staff.</li>
<li>Provider documentation is completed in Theoria's proprietary electronic health record (EHR) system, ChartEasy™. </li>
<li>PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems. </li>
<li>Provider is expected to be on-site on and on time for their in-person scheduled shifts. </li>
<li>Timely attendance is imperative.</li>
