Our team of highly skilled physical, occupational, speech, and respiratory therapists really care and work together to create an individualized therapy plan that helps you reach your highest level of functioning in as short an amount of time as possible - preparing for your eventual return home.

<li>Provide daily support to the unit leadership &amp; respond appropriately/timely to resident/family needs within the LPN scope of practice</li>
<li>LPN will use nursing process, judgment, &amp; skills to set priorities necessary to deliver quality direct resident care &amp; make decisions promoting the safety/quality direct care to resident's</li>
<li>Perform/administer standard nursing treatments and/or medications &amp; observes resident for desired or unwanted effects</li>
<li>Communicate &amp; collaborate with appropriate member(s) within the care team to revise/update the Nursing Care Plan or any pertinent findings including lab/test results, incidents, &amp;/or concerns</li>
<li>Monitor &amp; document vitals, treatments administered, &amp; residents' responses</li>
<li>Prepare, organize, oversee, monitor, &amp; evaluate delivery of care of resident CNA assignments &amp; activities, considering the category of needs &amp; competency of personnel</li>

 Requirements:

<li>Graduate of an approved school of Nursing</li>
<li>Licensed &amp; currently registered in good standing in New York as an LPN</li>
<li>Dedicated to providing extraordinary, clinically sound, &amp; compassionate care</li>
<li>Must possess a spirit of cooperation, caring &amp; enthusiasm to create an atmosphere conducive to rehabilitation &amp; optimal resident health</li>
