JOB SUMMARY: The Art Therapist serves to facilitate skills and expressive/art therapy groups and provide care coordination for patients. Specifically, the Art Therapist develops curriculum, prepares and facilitates daily skills and expressive therapy groups using art, media, imagery, and the creative process to help patients address social, emotional, psychological, and physical challenges in the milieu. Additionally, the Art Therapist educates and supports staff in understanding and facilitating groups as needed. ESSENTIAL DUTIES:

<li>Provide professional clinical services, including, but not limited to:

<li>Develop, implement and maintain skills group and expressive/art therapy group curriculum;</li>
<li>Facilitate group services utilizing Art Therapy interventions;</li>
<li>Perform&nbsp;psychosocial assessments and expressive therapy assessments;</li>
<li>Conduct treatment planning including creating and editing individualized treatment plans in the electronic medical record;</li>
<li>Provide care coordination, after care planning and case management services to assigned patients;</li>
<li>Provide crisis intervention;</li>
<li>Conduct family and individual therapy; and</li>
<li>Cross train to facilitate the intake process, including concurrent review process.</li>

<li>Collaborate with other treatment team members regarding treatment planning, progress, and ongoing needs in a timely and consistent manner.</li>
<li>Contact service providers and schools to obtain collateral information and provide mental health updates on current patients to increase continuity of care amongst professionals.</li>
<li>Aid the Bluestone Social Worker with completing assessments, discharge planning, care coordination and family communication.</li>
<li>Provides assistance and back up support to the inpatient social workers, Intake and other aspects of the inpatient program, as directed.</li>
<li>Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards.</li>

Job Overview
Work Schedule: Monday to Friday, from 8:00 AM to 4:30 PM, with a rotating on-call schedule during evenings and occasional weekends.

Compensation: Annual salary ranges from $116,480 to $135,200.
Nurse Practitioner Responsibilities
The Nurse Practitioner (NP), under the guidance of the Medical Director, delivers primary care to participants. Key duties include:
<ul>
<li>Conducting physical assessments for new PACE enrollees and performing semi-annual evaluations.</li>
<li>Creating and implementing tailored care plans for participants in the Senior Community Care program.</li>
<li>Assessing physical complaints and providing suitable treatments.</li>
<li>Educating participants and their caregivers.</li>
<li>Collaborating as a member of the Interdisciplinary Team (IDT).</li>
<li>Exhibiting the necessary knowledge and skills to care for frail elderly participants in accordance with established standards.</li>
</ul>
Required Qualifications
<ul>
<li>Must hold a valid Registered Nurse (R.N.) license in the state of employment.</li>
<li>Graduation from an accredited nursing program is essential.</li>
<li>Certification as a nurse practitioner from a nationally recognized certifying body is required.</li>
<li>A minimum of one year of experience working with the elderly and frail population is necessary.</li>
<li>Relevant training or experience related to the assigned role is required.</li>
</ul>
Preferred Qualifications
<ul>
<li>Certification as a Geriatric Nurse Practitioner (G.N.P.) with prescriptive authority is preferred.</li>
<li>Two years of experience as a nurse practitioner in a geriatric setting is desirable.</li>
</ul>
Core Responsibilities
<ul>
<li>Oversee residents in skilled nursing facilities.</li>
<li>Evaluate the health status of residents in skilled nursing facilities.</li>
<li>Provide medical care and treatment to residents.</li>
<li>Coordinate care with Primary Care Physicians and Medical Directors.</li>
<li>Conduct comprehensive history and physical examinations for new participants.</li>
<li>Work with the Interdisciplinary Team to develop comprehensive care plans.</li>
<li>Perform semiannual reassessments and additional evaluations as needed.</li>
<li>Integrate primary care treatment plans into the overall care strategy developed by the IDT.</li>
<li>Engage with team members to address urgent needs of participants and assist in discharge planning.</li>

$2000.00 sign on bonus for CCHT (Certified Clinical Hemodialysis Technician) or Bonent CHT (Certified Hemodialysis Technologist/Technician) Certifications.
 
Patient Care Technician (PCT) 
Patient Care Technicians (PCTs) at Health Systems Management support and work under the guidance of RNs to provide dialysis treatment to multiple patients at a time. PCTs have a combination of technical skills and compassion needed to improve the quality of life for our patients while promoting positive patient outcomes. Successful candidates will have the opportunity to learn the discipline of dialysis. To be considered for this position qualified candidates are required to have a minimum of 6 months of healthcare and/or direct patient care experience. Good fit for those with - CHT, CCHT, CNA, CMA, PCA, CPT, CMA, EMT, CST, CPhT, etc.

PCT Responsibilities and Physical Demands:
The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of the job.

<li>Set up of the hemodialysis machine. </li>
<li>Greet patients and collect details regarding their previous treatment. Report any concerns or observations to the lead nurse. </li>
<li>Assist with and prepare the patient's vascular access for treatment. </li>
<li>Initiate, monitor, and discontinue dialysis treatment per written policies, procedures, and standards of care. </li>
<li>Obtain patient's vital signs before, during, and after dialysis treatment and appropriately document in patient medical record. </li>
<li>Responsible for appropriately calculating and documenting patient's fluid removal. </li>
<li>PCT performs appropriate access cannulation and reports any difficulties or unusual findings to the lead nurse. </li>
<li>Monitor the patient for any adverse reaction(s) and conduct appropriate intervention.</li>
<li>Monitor the dialysis machine for any malfunction. Report any malfunctions to the lead nurse. </li>
<li>Demonstrates knowledge of infection control as related to hand washing, personal protective equipment (PPE), and universal precautions. </li>
<li>Maintain patient safety and comfort, which may include assisting with personal care needs and activities of daily living. </li>
<li>Responds appropriately to emergency situations. </li>
<li>Note you will have exposure to blood, needles, and various other bodily fluids. </li>
<li>This position provides direct patient care that regularly involves heavy lifting, moving of patients, and assisting with ambulation. </li>
<li>PCT will spend considerable time walking, bending, stretching, and standing. </li>
<li>The employee may occasionally be required to move machines, equipment, and may lift chemical and water solutions of up to 30 pounds and high as five feet. </li>
<li>You are required to acknowledge the inter-facility coverage policy that supports floating to various clinics during and after your training, if needed. </li>

PCT What You Can Expect: 
<ul>
<li>Improvement: HSM consistently strives to improve processes for our patients and teammates.</li>
<li>Training: May take plate in a facility or a different clinic other than your assigned home clinic. </li>
<li>Hours: Work hours may vary during the training period. However, you will typically work between 36-40 hours per week during training. </li>
</ul>
Education Requirements and Position Qualifications: 
<ul>
<li>High School Diploma required.</li>
<li>Basic computer knowledge required.</li>
<li>6 Months of healthcare and/or direct patient care experience is required.</li>
<li>Continued employment is dependent on successful completion of the Health Systems Management training program in its entirety. </li>
<li>The ability to work long hours is required. You could be on your feet for extended periods of time. Our day ends when patients’ care is completed.</li>
<li>You must have a flexible schedule and be able to work mornings, evenings, weekends, and holidays. </li>
<li>Must pass the Certified Clinical Hemodialysis Technician (CCHT) exam within 18 months following the first day of employment.</li>
<li>Must have or obtain Basic Life Support certification approved by the American Heart Association</li>
<li>Must be comfortable working around blood and needles, including cannulation, and obtaining labs.</li>
<li>Excellent bedside manner and communication skills. </li>
<li>Employee must successfully pass the Ishihara’s Color Blind Test.</li>
