SUMMARY:
Under the supervision of the Clinical Supervisor, the Clinical Therapist provides counseling and therapy services to clients with substance use disorder (SUD) and co-occurring mental health disorders. The Clinical Therapist conducts individual and group therapy under the structured framework of state and federal law, the counseling and/or social work code of ethics, SFRC/FCDRC policies and procedures.

Summary:

Essential Responsibilities
<ul>
<li>Performs and monitors laboratory operations to ensure quality control over analysis for diagnosis and treatment of patients, ensuring that laboratory testing systems and procedures provide quality laboratory services for patients.</li>
<li>Inventories, orders, and distributes laboratory supplies and equipment as needed;</li>
<li>Maintains a log and control system to minimize variances to workflows for speedier reports, and billing to patients and third parties;</li>
<li>Receives and logs laboratory requests; matches laboratory results with laboratory requests, and maintains the log for audit;</li>
<li>Records laboratory results for the provider and files laboratory results in patient medical record;</li>
<li>Performs venipuncture, lab procedures and records the results of laboratory tests in the patient’s record;</li>
<li>Performs waived laboratory testing as directed by a provider;</li>
<li>Performs phlebotomy and complete specimen processing;</li>
<li>Collects and labels specimens as requested by the provider; prepares specimens for analysis and completes requisition form/packages for pick-up as needed;</li>
<li>Performs routine maintenance on all analyzers, centrifuges, and similar lab equipment, uses critical thinking skills to trouble shooting problems; contacts repair and maintenance services as appropriate;</li>
<li>Calls patients to schedule blood draws, redraws and specimen re-testing;</li>
<li>Follows up, according to LFMC procedure, on normal/abnormal patient testing done by LFMC &amp; outside facilities, report findings in a timely manner to appropriate clinical staff, provide follow-up on diagnostic testing as indicated by provider.</li>

Requirements:
 Education, Experience and Skills/Training

<li>High school graduate or GED certificate and graduation from a technical school with appropriate degree and/or certification such as Laboratory Assistant/Aide program. Six months related experience is preferred; OR any equivalent combination of experience, training and/or education approved by Laboratory Manager or Chief Clinical Officer.</li>

Nurse Practitioner Duties
<ul>
<li>Provides patient care by examining and diagnosing patients, and carrying out or supervising medical treatment, therapy, and follow-up; referring patients for specialty consultation and treatment; educating patients in primary medical care; documenting patient-doctor transactions for proper record keeping and billing;</li>
<li>Assists and receives direction in the provision of patient care by consulting with LFMC physicians on the following situations, as needed:</li>
<li>when required by protocol;</li>
<li>when faced with a situation which may require hospital admission;</li>
<li>when incumbent is medically uncertain in a particular clinical situation;</li>
<li>when the clinical situation is beyond the scope of practice of the incumbent; </li>
<li>Conducts physical exams for all age groups to include health histories, labs, and other tests as appropriate with a focus on prevention; </li>
<li>Diagnoses and treats injuries and illnesses, both chronic and severe;</li>
<li>Prescribes medications, orders refills and maintains all required pharmacy logs;</li>

<li>Provides counseling to parents and/or children, focusing on preventive health care; educates patients by disseminating of patient information and literature relating to health education including but not limited to family planning, pre-natal care, and health promotion/disease prevention for all age categories;</li>
<li>Maintains compliance of clinic with state licensure by enforcing Quality Assurance protocols in the clinical, pharmacy, laboratory, medical supply, and nursing/patient care areas of LFMC for quality patient care;</li>
<li>Maintains and updates professional knowledge and skills, and maintains clinical proficiency by attending continuing education or seminars related to job responsibilities, including Age Specific Training (care giving, treatment, and assessments specific to the age of populations served). </li>

Requirements:
Education and Experience
<ul>
<li>This position requires that the Certified Nurse Practitioner be a graduate of an accredited Certified Nurse Practitioner program;</li>
<li>Preferred experience in a community health center, and working within a managed care environment.</li>

Licenses and Certifications

<li>Documentation of education, experience and licensure is required and must be maintained; Certified Nurse Practitioner must be insurable under the policies and requirements of LFMC; </li>

Nurse Practitioner

SUMMARY:
 
The DIS works as part of the interdisciplinary team to facilitate and coordinate clients’ continuum of care activities, ensuring optimum utilization of community resources, service delivery, and compliance with external agencies and referral sources requirements. The DIS facilitates disease intervention activities related to the prevention, screening, and treatment of sexually transmitted infections. The DIS works closely with community partners and state agencies to facilitate warm hand-offs and linkage to care. The DIS works in tandem with residential and outpatient case managers to provide wraparound services to clients in recovery. The DIS will report to the MAI Coordinator and work alongside the Case Manager Supervisors to maintain compliance with certifications and in-house procedures.

DUTIES AND RESPONSIBILITIES:
· Provide disease intervention and linkage to care services for clients who are at high risk for or living with HIV, Hepatitis C, or related sexually transmitted infections.
· Provide case management services including HIV prevention education, care coordination services, referrals to appropriate resources including infectious disease testing, HIV/HCV treatment, PrEP/PEP, community supports, and other assistance programs.
· Assess the needs of the clients at program entrance and work with the client to establish goals for prevention and treatment of sexually transmitted infections.
· Continue to follow up with clients throughout service provision to ensure that basic needs are addressed, including required documentation, to allow the client to move forward and achieve goals.
· Liaison with community services and other case managers to assist the client with identified goals and needs through recovery.
· Educate the clients on harm reduction, promoting healthy resources, and risk reduction training to provide highest quality of person-centered care.
· Uphold client confidentiality in accordance with company policies and procedures and local, state, and federal laws and regulations
· Maintains case files and other required documentation according to agency policies while working alongside residential and outpatient case managers, other members of the medical team, and the Prevention &amp; Recovery team.
· Ensure all GPRA documentation is completed in a timely manner
· Works with MAI Coordinator and grants department to ensure accurate data tracking, monitoring, and submissions to all appropriate agencies.
· May attend community-based events to promote health information and linkage to resources for prevention, screening, and treatment of sexually transmitted infections.
