Responsibilities

As a member of the interdisciplinary team (IDT) within any of the clinical areas, the LPN is responsible for performing direct patient care and medication administration under the supervision of the RN Case Manager and within the LPN scope of licensure in accordance with the Florida State Board of Nursing standards.

ADA: Full time position. Ability to sit for long periods of time and/or stand periodically during work day and lift up to 30 lbs., when necessary. Computer data entry a portion of job duty.
OSHA: Must adhere to universal precautions, to include blood borne pathogen protection, at all times
Position Summary: Provide and educate all aspects of patient care, including diagnosis, treatments and consultations. Work both inpatient and outpatient situations and can perform independently or as a part of treatment team. May conduct physicals, order tests and serve as a patient’s primary healthcare provider.
Role and Responsibilities:

<li>Help provide primary care to patients.</li>
<li>Perform physical exams and patient observations.</li>
<li>Must be proficient with medical instruments and equipment required by the work.</li>
<li>Knowledge of computer‐based data management programs and information systems, as well as records and point of interview technology.</li>
<li>Ability to communicate effectively, in verbal and written form, with retail and medical partners at various levels, patients, family members, physicians and representatives of the community.</li>
<li>Record patient medical histories and symptoms.</li>
<li>Create patient care plans and contribute to existing plans.</li>
<li>Order, administer and analyze diagnostic test.</li>
<li>Monitor and operate medical equipment.</li>
<li>Diagnose health issues, detect changes in a patient’s health and change treatment plans as necessary.</li>
<li>Administer medicines and treatments.</li>
<li>Evaluate responses to treatments and medications.</li>

Qualifications and Education Requirements:

<li>Master’s degree from an accredited nursing program.</li>
<li>Must have Florida RN Licensure and be board certified by either ANCC or AANP.</li>

Key Responsibilities

<li>Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.</li>
<li>Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.</li>
<li>Collaborate with the interdisciplinary care team to optimize patient outcomes.</li>
<li>Provide education and support to patients and families/caregivers on therapy programs.</li>
<li>Maintain accurate clinical records and report any changes in the patient’s condition to the physician.</li>
<li>Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.</li>

<li>Confirm daily visit schedules to ensure seamless coordination with other staff.</li>
<li>Communicate patient progress and changes with physicians and care teams.</li>
<li>Document all visits and progress notes on the same day of service.</li>

Qualifications 
<ul>
<li>Must have a current PT license (state-specific).</li>
<li>Graduate of an accredited physical therapy program.</li>
<li>Valid BLS certification.</li>
<li>Minimum one year of PT experience (home health experience preferred).</li>
<li>Excellent communication, organizational, and time management skills.</li>
<li>Must have a valid driver’s license and access to a reliable and insured vehicle.</li>
<li>Ability to visit homes that may not be wheelchair accessible.</li>
<li>Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.</li>
<li>Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.</li>
<li>Proficiency with electronic medical records is a plus.</li>
