The Medical Assistant will manage the customer flow throughout the clinic and assist providers in managing and treating their patients. The focus of the Medical Assistant is on collaborative, supportive, and delegated tasks as described in the Blaze Health Medical Assistant scope of practice. Delegable tasks and competencies are specific to the role of the Supervising Nurse and Supervising Provider and are described by the Minnesota Board of Nursing and the Minnesota Board of Medical Practice. The Medical Assistant receives direction and collaborates with the Clinic Supervisor, Clinic Manager, and Providers to meet the defined goals of the customer, shift, department, and organization. Communication with the health care team related to progress, concerns, questions, or problems that arise are an essential element of the Medical Assistant role.
 
Key Responsibilities:

 A collaborating physician will be provided; however, they may not specialize in mental health disorders.

&nbsp;RESPONSIBILITIES:&nbsp;As a Mid-Level Provider with Primary Care Solutions, you'll have the opportunity to serve our nation's Veterans with the same exceptional level of care our Veterans have shown while serving our country. <li>Examine the patient/client and determine the best course of action while documenting in our paperless health program – The VA’s VISTA/CPRS electronic patient record system.</li>
<li>Develop and implement patient management plans, communicating with Patient Aligned Care Team (PACT) to ensure appropriate standards of care.</li>
<li>Discuss patient outcomes, policies, procedures, and records with Medical Director.</li>
<li>Completes all clinical reminders to keep each patient up to date in the system.&nbsp;</li>
</ul>
&nbsp;JOB REQUIREMENTS:&nbsp;
<ul>
<li>Graduate of an accredited school of nursing and graduate of an accredited program for nurse practitioners (MSN required).</li>
<li>Board-certified in family practice or internal medicine</li>
<li>Certification by an applicable professional organization</li>
<li>1 year of clinical medical practice preferred</li>
<li>Current, unrestricted license in the states where PCS operates</li>
<li>Current BLS for providers from the American Heart Association (to be renewed annually)</li>
<li>Current, unrestricted Drug Enforcement Administration (DEA) registration</li>
<li>Excellent computer skills</li>

Essential Functions &amp; Responsibilities
<ul>
<li>The physician serves as the attending physician for the residents residing at each facility;</li>
<li>The physician collaborates with Nurse Practitioners and Physician Assistants to manage day to day resident care at each facility. Typically, each facility will have a full-time PA or NP assigned to the facility depending on the acuity and volume of the patient population;</li>
<li>Physician will provide high quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs;</li>
<li>Physician is responsible for diagnosing and treating patients with acute illness, such as bacterial and viral infections, as well as chronic diseases and conditions, such as hypertension, heart disease, or diabetes</li>
<li>Provider documentation is completed in Theoria's proprietary electronic health record (EHR) system, ChartEasy™</li>
<li>PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems</li>
<li>Participating in Theoria's Chronic Care Management, Transitional Care, RPM and other medical quality initiatives and programs</li>
<li>Physician must be able to supervise collaborating nurse practitioners and physicians assistants</li>
<li>Physician must be able to utilize electronic/online medical tools such as EHR systems and state/county medical systems utilized for Death Certificates and administrative functions.</li>

Shift Structure
<ul>
<li>Shifts are flexible depending on physicians' schedule;</li>
<li>Typically, attending physicians work a minimum of 1-2 days/week at each facility they oversee;</li>
<li>Rounding times are flexible with no set start and end time. Preferably, physicians start in the morning between 7-10 a.m. and finish their day when their caseload has been seen;</li>
<li>No nights, weekends, or on-call</li>
<li>Call is covered by Theoria's virtual care team</li>
<li>This is an in-person position</li>
