Under the supervision of the Director of Nursing, the Registered Nurse oversees the day-to-day nursing functions of an assigned area to include supervising the Certified Nursing Assistants and providing nursing care to residents as prescribed by the physicians and in accordance with standards of nursing practices and regulations.
 
Job Duties:
<ul>
<li>Supervise other professional and non-professional staff in the day-to-day delivery of resident care; schedule job assignments and develop nursing unit priorities; orient new staff and participate in the recruitment and selection of nursing personnel; communicate policies, assist and coach as needed; monitor work assignments, provide feedback, evaluate performance, and redirect and discipline employees as needed.</li>
<li>Prepare and review medical records and nurse’s notes and assess residents’ physical and emotional status; monitor resident activity and provide nursing care according to the physician’s orders, care plans, established standards and facility policies; coordinate and assist with new admissions, transfers, and discharges; transcribe physicians telephone orders; communicate with residents, family members, other interdisciplinary team members, and management regarding resident’s status.</li>
<li>Prepare and administer medications as ordered by the physician in accordance with nursing standards and facility policies; sign, date and perform all charting and recordkeeping in accordance with established policies and procedures.</li>
<li>Understand, comply with, and promote all rules regarding residents’ rights.</li>
<li>Assist and participate in developing and implementing written care plans for individual residents that address the needs of the resident.</li>
<li>Participate in in-service training; assist in implementing orientation programs and job skills training; maintain professional competence.</li>
<li>Monitor environment and care practices of nursing personnel to ensure compliance with established safety, fire, disaster, infection control and all other departmental policies and procedures; ensure cleanliness and safety of work and treatment areas.</li>

<li>Must be a Registered Nurse (RN) in good standing and currently licensed by the State.</li>
<li>Must be able to relate positively to residents and families and work cooperatively with other employees.</li>
<li>Must be capable of maintaining regular attendance.</li>
<li>Must meet all local health regulations, and successfully complete a post-offer health assessment.</li>
<li>Must be capable of performing the essential functions of this job, with or without reasonable accommodation.</li>
<li>Experience as a Charge Nurse or supervisory experience preferred.</li>
<li>Physical and Sensory Requirements (with or without the aid of mechanical devices): Walking, reaching, climbing, bending, lifting, grasping, fine hand coordination, pushing and pulling, ability to hear and respond to pages; ability to distinguish smells, tastes and temperatures, ability to read and write, ability to communicate both verbally and in writing with residents, families, personnel, vendors, and consultants; ability to remain calm under stress.</li>

We have a wide variety of roles available suited for providers in various stages of their careers with flexible scheduling in a positive work environment. 
Shift Structure

<li>In-person rounding shifts start around 7AM local time with flexibility within reason;</li>
<li>Shifts are typically available Saturday and Sundays and major holidays </li>
<li>This role is responsible for covering weekend needs and two holidays per calendar year</li>
<li>Charting/documentation can be completed off-site</li>
<li>This is an in-person position </li>

Essential Functions and Responsibilities 
<ul>
<li>Provider is responsible for rounding and managing the clinical care of the population at the assigned SNF </li>
<li>The provider is responsible for rounding daily and prescribing medications and treatments for residents residing at SNFs </li>
<li>Provider will provide high quality, patient-centered, comprehensive primary care services to patients residing in SNFs </li>
<li>Provider is responsible for timely documentation of all patient care/encounters </li>
<li>The provider will be available for phone/secure messaging consultations with facility nursing staff</li>
<li>Provider documentation is completed in Theoria's proprietary electronic health record (EHR) system, ChartEasy™ </li>
<li>PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems </li>
</ul>
Physical Requirements
<ul>
<li>Must be able to move inside a facility frequently and briskly in order to see patients, meet with managers and facility staff, and attend to patient needs.</li>
<li>Punctual in-person attendance (typically between 7 a.m. and 1 p.m. is required (for rounders).</li>
<li>Continuous operation of app-based EMR/notes system either through a computer or on a mobile device (iPad, Phone etc.).</li>
<li>Ability to ascend and descend stairs in a facility.</li>
<li>Some ability to position oneself to obtain records, tools, medical equipment and other items that may be in locations that are below the knee or above the shoulder.</li>
<li>Effectively communicate with patients, managers, and Facility staff.</li>
<li>Ability to observe details in close range (within a few feet of the observer) in order to provide accurate and appropriate medical recommendations, treatment, and to provide timely updates to Managers regarding facility or patient concerns.</li>
<li>Must be able to lift or move at least 25 pounds.</li>

Requirements and Qualifications
<ul>
<li>Candidate must hold a valid PA/NP license in the state in which they will practice </li>
<li>Candidate must hold a valid NP/PA degree from an accredited institution</li>
<li>Candidate must have an active DEA license (If new graduate, Theoria will assist with your application) </li>
<li>General Post-Acute/SNF knowledge or experience preferred </li>
<li>Candidate must be familiar with EMR systems and charting practices </li>
