QUALIFICATIONS:
1. Current LPN Minnesota state license, current Medical Assistant certification or registration required. Must maintain license/certification/registration. 2. Job Description
Our Cook Carefree Living facility is looking for a Clinical Supervisor/RN to join our growing and established Memory Care and Assisted Living community in Cook, MN. Schedule: M-F 8am-430pm based on the needs of the facility
(we have an outside after-hours triage that we provide to all our facilities)
RESPONSIBILITIES: The Director of Nursing is responsible for clinical and departmental functions of the assisted living and memory care communities they manage. Main responsibilities include:
<ul>
<li>Ensure quality of care and regulatory compliance by providing over-site and direction to all levels nursing staff.</li>
<li>Responsible for implementing the nursing process, including assessment, case management, diagnosis, planning, goal setting, implementation and evaluation, in the delivery of professional and paraprofessional services according to MDH licensing.</li>
<li>Meet with the client and family to accurately complete all admission paperwork, explaining the home care plan and client/family responsibilities. Participate in care conferences as necessary.</li>
<li>Monitor client's funding sources to assure that prior authorization is obtained and transferred accurately and timely to the prior Authorization Department and that the appropriate payors are billed. Assist with collections as necessary.</li>
<li>Supervise and coordinate maintenance of appropriate, accurate and complete documentation of information on clients and staff under your supervision according to agency policy.</li>
<li>Delegate appropriate nursing functions according to State Licensure and agency policies and procedures. Maintain compliance with current county, state and federal regulations affecting home health care.</li>
<li>Actively participate in agency-wide activities and its continuous quality improvement program. Responsible for evaluating the home care program's level of excellence and implementing process improvement strategies as indicated.</li>
<li>Develop and implement safety programs to ensure the well-being of residents, staff and visitor as well as avoiding injuries and financial loss. </li>
<li>Perform other functions as deemed appropriate by the Executive Director or Regional Manager of Operations.</li>
<li>Market the wide range of health care services to potential clients and their families. Assist with/support marketing strategies to achieve financial stability through maximum census. Network with other community providers.</li>
<li>Develop and maintain relationships with third parties such as county social workers, hospital discharge planners and others to assist in quality care and referral issues. Network with other community providers.</li>
<li>Participate in the annual clinical budgeting process and work with the Executive Director in developing new and creative ways to grow revenue and maximize operational profits to achieve the organization's operating and financial objectives. </li>
<li>Monitor budget expenditures with nursing personnel throughout the year.</li>
</ul>
QUALIFICATIONS:
<ul>
<li>Current RN licensure with assisted living housing experience preferred.</li>
<li>CPR/BLS required.</li>
<li>Must have a passion for working with seniors and providing excellent care and customer service.</li>
<li>Excellent observation, verbal and written communication skills, and problem-solving skills</li>
<li>Position requires computer use; knowledge of Internet and Microsoft Office is required.</li>
<li>Must have Drivers' License in good standing and the ability to drive to other locations.</li>
<li>Selected candidate must be able to meet the physical requirements of the position and will be subject to a DHS background study.</li>
