Role Overview
Hale Kipa is looking for a Wraparound Facilitator/Care Coordinator to facilitate and coordinate wrapround meetings and services. The Wraparound Facilitator/Care Coordinator is responsible for overseeing the development of the wraparound team and orienting team members to the wraparound process. The Wraparound Facilitator/Care Coordinator coordinates and collaborates with wraparound staff, families and youth, service providers, and community providers to complete the Youth and Family Wrapround plan and Safety /Crisis plan. Function
The Seasonal Lead Cold Weather Case Manager position supports the provision of Coordinated Entry of social services in Colorado by providing information, referral, intake assessment, and emergency casework services over the phone, via email, video chat, and/or in person. The Cold Weather Team will be responsible for providing access to shelter during severe weather events in alignment with citywide severe weather policies. This will include expanding the hours of operation and/or operating an additional telephone line to provide additional access during severe weather events. This also includes working with HOST on a procedure to provide additional hours of operation and additional staff coverage during distinct weather events. Additionally, The Salvation Army will be responsible for providing on-site staffing at the Cold Weather sheltering location during activation, to include screening families for eligibility, providing rapid resolution services as appropriate, writing vouchers on the spot for cold weather, and completing HMIS intakes and data entry.

<li>Collaborate and participate in clinical team meetings and ongoing training</li>

<li>High-school diploma or equivalent</li>
<li>Must be at least 18 years of age (21 years or older if located in New Jersey)</li>
<li>Availability during after-school hours, weekends, and holidays</li>
<li>Consistent, reliable transportation, a valid driver’s license, and car insurance to provide in-home services for clients</li>
<li>Must maintain clean background/drug screenings and driving record</li>
<li>BT: Ability to attend our 40-hour RBT fulltime paid training course and be eligible to pass the RBT exam within 45 days of hire</li>
<li>RBT: Ability to attend required paid training program</li>

QUALIFICATIONS:
