<li>Monitoring and tracking client progress.</li>
<li>Empowering clients to actively participate in their own planning and goal setting.</li>
<li>Conducting regular meetings with clients (weekly/bi-weekly or as needed).Referring clients for job development opportunities if appropriate.</li>
<li>Connecting clients with suitable resources to help them achieve their goals.</li>
<li>Meeting with clients to address challenges, successes, and strengthen their capacity for long-term self-sufficiency.</li>
<li>Maintaining thorough client files, documenting conversations, warnings, and progress reports, while preserving confidentiality.</li>
<li>Reporting critical incidents immediately to the Site Director/Director of Social Services.</li>
<li>Collaborating closely with other Housing Specialists and Employment staff to identify client readiness for employment and housing.</li>
<li>Treating all clients, visitors, and employees with kindness, respect, and dignity.</li>
<li>Serving as an advocate for clients when necessary.</li>
<li>Providing periodic case management and counseling, extending one year beyond the client's departure.</li>
<li>Establishing and nurturing connections with other programs and units within the organization to facilitate inter-agency referrals.</li>
<li>Escorting clients to housing viewings, DHS or HRA appointments as required.</li>
<li>Facilitating suitable housing placements for clients.</li>
<li>Ensuring that case notes are completed within 24 hours after client meetings.</li>
<li>Offering support as a substitute for other staff when needed.</li>
<li>Performing general clerical duties.</li>
</ul>
 
Physical Requirements:
<ul>
<li>Prolonged periods of sitting at a desk and working on a computer.</li>
<li>Ability to sit for extended periods and perform repetitive tasks.</li>
<li>Must be able to lift and carry up to 5 pounds.</li>
<li>Must be able to travel to multiple NYC sites as needed.</li>
<li>Must be able to access and navigate each department at the organization’s facilities.</li>
<li>Ability to climb stairs.</li>
</ul>
 
Work Environment / Schedule Requirements:
<ul>
<li>Office setting with regular exposure to computer screens and moderate noise levels.</li>
<li>May be required to work shifts, including evenings or night tours and/or Saturdays, Sundays and holidays with holiday pay differential.</li>
<li>Transitional Housing Program staff are required to continue their shift until relieved by another staff member or authorized to leave by a supervisor.</li>
</ul>
 
Qualifications:
<ul>
<li>Bachelor’s degree in social work, psychology, sociology, or a related field with at least 2 years of direct social service experience.</li>
<li>Associate degree in human services or a related field with a minimum of 4 years of direct social service experience.</li>
<li>High School Diploma/General Equivalent Diploma with a minimum of 8 years of direct social service experience.</li>
<li>Exceptional interpersonal skills, with the ability to balance compassion and firmness while always maintaining confidentiality.</li>
<li>Proficiency in navigating community resources.</li>
<li>Strong problem-solving abilities, decision-making skills, conflict resolution expertise, and active listening.</li>
<li>Capacity to remain composed in crisis situations.</li>
<li>Demonstrated ability to collaborate effectively with diverse groups.</li>
<li>Proven capability to manage multiple tasks efficiently under pressure.</li>
<li>Outstanding organizational skills, attention to detail, and efficiency.</li>
<li>Possessing maturity, integrity, and sound judgment.</li>
<li>Must be able to read, speak, write, and understand English for administrative purposes.</li>
<li>Must pass drug screening to be appointed.</li>
<li>This position may be subject to a series of investigations before and after appointment.</li>
