<li> Develop and implement individualized treatment plans in collaboration with the supervising physician, focusing on health promotion, disease prevention, and management of acute and chronic conditions. </li>
<li> Prescribe medications, order therapies, and manage medical devices according to state regulations and collaborative practice agreements. </li>
<li> Provide patient education and counseling regarding health maintenance, disease management, medication adherence, and lifestyle modifications. </li>
<li> Evaluate and manage common acute illnesses (e.g., upper respiratory infections, influenza) and chronic conditions (e.g., hypertension, diabetes, asthma). </li>
<li> Perform minor office procedures as trained and within scope of practice. </li>
<li> Collaborate effectively with physicians, nurses, medical assistants, and other members of the healthcare team to ensure coordinated and comprehensive patient care. </li>
<li> Document all patient encounters accurately and thoroughly in the electronic health record. </li>
<li> Participate in quality improvement initiatives and adhere to all organizational policies and procedures. </li>
<li> Serve as a resource for medical staff and participate in educational activities as needed. </li>
<li> Provide age-appropriate care to patients across the lifespan, from infants to geriatrics. </li>
<li> Manage referrals to specialists and coordinate follow-up care. </li>
<li> Maintain current knowledge of family medicine best practices and engage in ongoing professional development. </li>

 Qualifications: 
<ul>
<li> 1-2 years clinical experience in a of experience in primary care/internal medicine setting </li>
<li> Active PA or NP in family medicine specialty certification. </li>
<li> Active, unrestricted CT medical license or in process of obtaining </li>
<li> ACLS/BLS/PALS (as required) Certification </li>
<li> DEA License </li>

 Schedule: 40 scheduled hours, M-F, 8am-5pm, 4-day work week encouraged Skill Set: Prefer 1-2 years in a clinical working experience as a NP/PA within the given specialty/service line recommended.
