Program Overview:

Intercept®, developed by Youth Villages, is an evidence-based intensive, in-home program that focuses on keeping families together by preventing out-of-home placements or facilitating reunification after such placements, including foster care, residential treatment facilities, hospitalization, or juvenile detention centers. It is one of the first programs to receive the well-supported designation from&nbsp; Title IV-E Prevention Services Clearinghouse developed in accordance with the Family First Prevention Services Act (FFPSA) having been shown to reduce out-of-home placements and accelerate permanency for children. This program serves youth from birth to age 18 who are facing serious emotional or behavioral challenges or have experienced trauma, such as abuse or neglect. Services are provided by highly trained Family Intervention Specialists directly in the family’s home and community–Intercept® is not an office-based program. Intercept provides intensive, strength-based support tailored to each family’s unique needs, ultimately helping them achieve lasting change through evidence-based mental health interventions, along with the development of new parenting and communication skills.

Essential Duties and Responsibilities: 

The Family Intervention Specialist:

Carries a caseload of 4 to 6 families
</li>
<li>
Holds individual and family sessions with each family 3 times a week, scheduled at the convenience of the families
</li>
<li>
Conducts on-going assessment of youth to determine their needs from a strength-focused, solution-based perspective
</li>
<li>
Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development to help enhance clinical skills
</li>
<li>
Drives up to 60 to 80 miles to meet with families in their homes and communities (additional travel may be required)
</li>
<li>
Collaborates with other providers, case workers, and courts to formulate a collaborative treatment plan
</li>
<li>
Provides on-call crisis support to the youth and family (schedules vary by location)
</li>
<li>
Completes accurate and timely documentation in an electronic medical record system (EMR)
</li>
<li>
Performs other duties as assigned
</li>

Schedule is flexible and non-traditional as it is based around the availability of youth and families served.

$56,000-$60,000 per year based on education and clinical license

2. Data Analysis and Business Intelligence

5. Leadership Support and Coordination

<li>Provide senior leaders with timely EXSUMs, information papers, smart books, and program documentation (including four-hour responses when required).</li>
<li>Coordinate with ASA(ALT), PMs, HQDA staff, ACOMs, and Joint organizations on equipping and modernization issues.</li>
<li>Prepare briefing products and support decision forums (boards, councils, reviews).</li>
</ul>

6. Advanced Analytics and Modeling
<ul>
<li>Develop statistical, predictive, and simulation-based models (e.g., regression, ANOVA, queuing theory, flow modeling).</li>
<li>Collect and organize data from multiple sources, apply analytic techniques, and determine measures of effectiveness.</li>
<li>Construct symbolic models of processes/systems and apply computational, mathematical, and statistical techniques to evaluate solutions.</li>
<li>Use predictive modeling, cost analysis, and business reengineering/IDEF modeling to solve complex resourcing and modernization problems.</li>
</ul>

7. Strategic Communication and Documentation
<ul>
<li>Produce decision-support briefings, budget justification documents, and strategic communication plans.</li>
<li>Maintain situational awareness of assigned programs and report funding, readiness, and equipping status to leadership.</li>
<li>Conduct quality reviews of program materials to ensure accuracy and clarity for senior decision-makers.</li>
</ul>

Minimum Qualifications
<ul>
<li>Bachelor's degree in operations research, applied mathematics statistics, engineering (Systems or Industrial), or computer science</li>
<li>A minimum of four (4) years ORSA experience</li>
<li>Secret Clearance required</li>
<li>Ability to demonstrate knowledge and experience in identifying and solving real-world problems in areas such as capital investment, decision analysis supporting a business unit, system design and engineering, linear and non-linear programming, and trend/predictive analysis</li>

Responsibilities
The Licensed Practical Nurse (LPN) plans, coordinates, provides and manages nursing care services and health education to nursing home guests.
<ul>
<li>Supervise the care/services provided by Certified Nursing Assistants (CNA)/State-Tested Nursing Assistants (STNA) and other team members who care for guests.</li>
<li>Provide safe and accurate medication-related interventions to guests.</li>
<li>Assess the health of guests and notify the physician of changes in status. Promptly implement new orders.</li>
<li>Select and institute appropriate nursing interventions to stabilize a guest’s condition and/or prevent complications.</li>
<li>Contribute to the guest’s assessment (MDS/CAA’s) and the development of a plan of care.</li>
</ul>
Qualifications
<ul>
<li>1-3 years of experience in a long-term care setting preferred.</li>
<li>Current Licensed Practical Nurse (LPN) licensure in the state.</li>
<li>CPR certification.</li>
<li>or acceptable exemption required.</li>

<li>Participate in monthly meetings led by the Clinic Director and Office Manager to identify challenges within the clinic to address and improve clinic flow.</li>
<li>Attend company-wide meetings to discuss various company initiatives to continuously improve clinical excellence and patient care.</li>
