Meadow Park Rehabilitation &amp; Healthcare Center is actively seeking an RN to manage our modern, newly renovated subacute unit.As a charge nurse you will be responsible for:
<ul>
<li>Managing the individualized resident care by promoting and monitoring residents' health through the nursing process.</li>
<li>Utilizes clinical resources in implementing patient care and provides rationale for patient care decisions.</li>
<li>Collaborating with physicians and interdisciplinary team members to resolve all residents' problems and needs</li>
<li>Providing physical and psychological support to patients and their family members</li>
<li>Monitoring residents' conditions and administering medication &amp; treatments</li>
<li>Supervising assigned team members.</li>
<li>Completes Frequent Observations and the Intake/Outtake Record according to established protocols.</li>
<li>Performs related duties, as required.</li>
</ul>
Requirements:
<ul>
<li>Licensed as a Registered Nurse in the State of NY</li>
<li>Past experience working in a Long Term Care Nursing Facility</li>
<li>Knowledge of EMR systems</li>
<li>BCLS, ACLS or PALS, required.</li>
<li>Excellent communication skills</li>
