Centinela Hospital Medical Center is seeking a Director of Case Management to lead a high-performing team and drive exceptional patient care, outcomes, and operational efficiency.
 
What You’ll Do
<ul>
<li>Lead and develop a team of RNs, Social Workers, and Case Management staff</li>
<li>Oversee utilization management, discharge planning, and care coordination</li>
<li>Ensure patients receive timely, appropriate, and cost-effective care</li>
<li>Drive initiatives to improve patient outcomes and reduce readmissions</li>
<li>Use data and quality improvement tools to optimize performance</li>
</ul>
 
What We’re Looking For
<ul>
<li>Active California RN license (required)</li>
<li>BSN required (MSN or advanced degree a plus)</li>
<li>OR LCSW with 5+ years post-grad experience (if applicable)</li>
<li>5+ years in Case Management</li>
<li>Strong analytical skills and experience with utilization review</li>
<li>Knowledge of InterQual / Milliman criteria preferred</li>
