POSITION DESCRIPTION
I. Position: Intake and Consumer Services Coordinator
II. Status: Full Time, Hourly
III. Reports to: PAS Program Director
IV. Position Summary: Responsible for Personal Assistant Services (PAS) consumer intakes and referrals. Assists with the coordination of consumer services.
V. Position Expected Hours of Work: This is a full time position. Days and hours of work are Monday through Friday 8am to 4:30pm. Occasional evenings and weekends.
VI. Essential Functions of the Position:
- Responsible for consumer intakes and referrals including verification of eligibility, both phone and in person, maintaining confidentiality and repect to consumer needs. Ability to triage for appropriateness for PAS services.
- Provide education to consumer and caregivers on eligibility for services and systems navigation
- Complete required documentation in PAS deatabases; complete reports on related activities and outcomes as designated.
- Activerly participate in the PAS Coordination team meetings and other meetings as requested
- Communicate effectively with internal teams and external partners.
- Attend community events to support recruitment and engagement efforts, acting as an ambassador for Independence First
- Advocate for consumer needs with outside partners where appropriate
VII. Work Environment
- The work environment is generally performed in an office setting with minimal to moderate noise level.

I. Professional Qualifications
- Degree in a related field or equivalent work experience
- CNA level experience required
- 1-2 years of experience in project development and management as well as demonstrated success in reaching set outcomes.
- Demonstrated training and experience in determining Activities of Daily Living competency levels a must
- Knowledge base of personal cares: ADL's and IADLs and ability to assess and to provide training of this knowledge required.
- Must have a firm belief in consumer control, the independent living philosophy and our agency Vision Statement.
- Excellent communication skills to include speaking, writing, listening and relationship building.
- Strong time management, organization and prioritization skills.
- Attention to detail and accuracy a must
- Excellent customer service skills
- Strong analytical problem solving and critical thinking skills.
- Ability to work both independently and cooperatively as part of a team with minimal supervision.
- Flexibility in the work environment and willingness and ability to adapt to changing departmental/agency needs.
- Positive attitude and professional approach to work.
- Knowledge of community based resources, intake and referral experience.
- Ability to handle confidential information and confrontational situations
- Ability to effectively listen to concerns of workers and consumers and problem solve for an equitable resolution
- Must be able to visit homes that may or may not be accessible
- Knowledge of Medicaid, Managed Care, Family Care, IRIS desired
- Demonstrated Computer and software skills required; proficiency with Microsoft office.
