Title: Licensed Practical Nurse
Employment Type: Contract (3+ Months)
Compensation Range: $36.00–$40.00 per hour
Location: Liberty, New York
Work Schedule: Onsite; 7:00 AM–3:00 PM or 3:00 PM–11:00 PM, 5 days per week, every other weekend; overnight availability preferred

Position Summary
The Licensed Practical Nurse provides professional nursing care to residents in accordance with individualized care plans under the direction of a Registered Nurse. The position requires sound clinical judgment, effective communication, accurate documentation, and supervision of Nursing Assistants while ensuring quality resident care.
Key Responsibilities
- Provide professional nursing care according to individualized care plans.
- Administer prescribed medications and therapeutic treatments in accordance with physician orders.
- Observe, assess, and document resident conditions and report changes appropriately.
- Measure and record vital signs.
- Collect laboratory specimens as required.
- Maintain accurate nursing documentation and medical records.
- Communicate effectively with physicians, Registered Nurses, residents, and families.
- Assist with resident admissions, transfers, and discharges.
- Supervise Nursing Assistants and assigned clerical personnel.
- Ensure compliance with facility policies and applicable nursing standards.
Required Qualifications
- Active Licensed Practical Nurse (LPN) license in New York State.
- Ability to provide direct resident care in a long-term care setting.
- Knowledge of medication administration and nursing procedures.
- Strong communication and documentation skills.
- Ability to supervise support staff.
- Ability to work every other weekend.
Preferred Qualifications
- Previous skilled nursing or long-term care experience.
- Experience with electronic medical records.
- Strong assessment and clinical documentation skills.
- Flexible availability, including overnight shifts.
