Advanced Care is seeking a motivated Community Clinical Liaison to support our skilled nursing and rehabilitation centers throughout the Evansville market. This position is responsible for building referral relationships, increasing admissions, and serving as a trusted resource to hospitals, physicians, case managers, social workers, and other healthcare partners.

Responsibilities

• Develop and maintain relationships with hospitals, physician offices, assisted living communities, and other referral sources.

• Increase admissions and census through strategic business development efforts.

• Evaluate referrals and coordinate admissions with facility teams.

• Educate referral partners on skilled nursing, rehabilitation, and post-acute care services.

• Participate in community outreach and networking events.

Qualifications

• 2+ years of experience as a Clinical Liaison, Hospital Liaison, Admissions Director, Director of Marketing, or related healthcare sales role.

• Skilled nursing, rehabilitation, home health, hospice, or post-acute care experience preferred.

• Strong communication, relationship-building, and organizational skills.

• Knowledge of Medicare, Managed Care, and insurance authorization processes preferred.

• Reliable transportation and ability to travel throughout the Evansville market.
