Disability Services of the Southwest (DSSW) is seeking a passionate and dedicated DBMD Case Manager to support individuals with deaf-blindness and multiple disabilities in achieving greater independence, community inclusion, and quality of life. This position plays a critical role in coordinating services, advocating for clients, and ensuring compliance with program standards within the Deaf Blind Multiple Disabilities (DBMD) waiver program.

Position Summary

The DBMD Case Manager is responsible for the day-to-day management of assigned client caseloads and the internal operation of the DBMD waiver program. This role ensures that services are delivered in accordance with state regulations and program guidelines while maintaining strong relationships with clients, families, providers, and community partners.

Key Responsibilities

  • Manage daily activities and operations of assigned DBMD program participants.
  • Monitor service delivery and ensure quality outcomes for assigned caseloads.
  • Coordinate enrollment and onboarding of eligible applicants.
  • Ensure compliance with DBMD program requirements, regulatory standards, and licensure guidelines.
  • Advocate for clients and safeguard individual rights.
  • Collaborate with families, service providers, healthcare professionals, and community stakeholders.
  • Participate in required trainings, team meetings, and ARD meetings as applicable.
  • Provide crisis intervention and problem resolution when needed.
  • Maintain accurate and timely documentation.
  • Support on-call responsibilities and client care activities as assigned.
  • Travel throughout the Austin service area for client visits, assessments, and community-based activities.
  • Maintain strict confidentiality and comply with all federal, state, and company policies.
  • Perform additional duties as assigned.

Qualifications

Education & Experience

Candidates must meet one of the following requirements:

  • Bachelor's degree in a health or human services-related field and at least two years of experience providing direct services to individuals with disabilities; OR
  • Associate degree in a health or human services-related field and at least four years of relevant experience; OR
  • High school diploma (or equivalent) and at least six years of experience providing direct services to individuals with disabilities.

Preferred Skills & Abilities

  • Strong case management and advocacy skills.
  • Ability to work independently while collaborating effectively with a multidisciplinary team.
  • Excellent organizational, time management, and problem-solving abilities.
  • Strong written and verbal communication skills.
  • Ability to build positive relationships with clients, families, staff, and community partners.
  • Experience working with individuals with disabilities preferred.
  • Proficiency with Microsoft Office applications and database systems.

Additional Requirements

  • Ability to become fluent in the communication methods used by assigned individuals, including American Sign Language (ASL), tactile communication, communication boards, gestures, or other individualized methods.
  • Valid Texas driver's license, reliable transportation, and current auto liability insurance.
  • Current CPR and First Aid certification (in-person certification required).
  • Successful completion of required agency and HHSC training programs.
  • Ability to type at least 35 WPM and effectively utilize electronic documentation systems.
  • Must pass all required background screenings and employment eligibility checks.

Physical Requirements

This position requires frequent travel, community-based work, mobility throughout various environments, and the ability to assist individuals as needed. Physical activities may include standing, walking, climbing stairs and ramps, lifting up to 50 pounds or more, pushing/pulling wheelchairs, bending, reaching, and transferring individuals safely when required.
