Location: On-site – Chicago & Park Forest, IL
Chicago Location: 8640 S. South Chicago Avenue, Chicago, IL 60617
Schedule: Full-Time | 40 hours per week
Pay: $18.00–$20.00 per hour

Aunt Martha’s Health & Wellness is seeking an organized, detail-oriented professional to join our team as Central Matching Team Support Staff.

This is a great opportunity for someone with experience in administrative support, healthcare, human services, social services, case coordination, referrals, or data management who enjoys keeping information organized and helping teams work together.

You’ll play an important behind-the-scenes role supporting programs that serve children and families involved with the Illinois Department of Children and Family Services (DCFS). Your work will help ensure referrals are processed accurately, information reaches the right people, and cases continue moving forward.

Previous DCFS experience is helpful, but not required.

What You’ll DoReferral Coordination & Communication

  • Coordinate incoming referrals and ensure required information and documentation are complete.

  • Communicate with casework staff, provider agencies, internal teams, and DCFS partners.

  • Help prioritize urgent and emergency referrals.

  • Monitor referral activity and follow up when additional information is needed.

  • Provide backup support for centralized email communications and referral correspondence.

Data Entry, Tracking & Reporting

  • Enter and maintain referral and case information across multiple electronic systems.

  • Review information for accuracy and completeness.

  • Maintain organized electronic documentation and records.

  • Generate, review, and assist with reports used to track program activity.

  • Help identify missing information, discrepancies, and outstanding follow-up items.

Administrative & Program Support

  • Provide day-to-day administrative support to the Central Matching Team.

  • Assist staff with referral systems, documentation, and data-management processes.

  • Help monitor referral workflows and matching activities.

  • Participate in meetings, trainings, and quality-improvement initiatives.

  • Support special projects and other administrative needs as assigned.

What We’re Looking For

We’re looking for someone who is dependable, organized, comfortable working with data, and able to communicate professionally with a variety of people.

Required Qualifications

  • High school diploma or GED.

  • Proficiency with Microsoft Word, Excel, and Outlook.

  • Strong written and verbal communication skills.

  • Strong organizational skills and attention to detail.

  • Ability to manage multiple priorities and deadlines.

  • Comfortable entering, reviewing, and maintaining information in electronic systems.

  • Ability to maintain confidentiality and handle sensitive information professionally.

Preferred Qualifications

  • Bachelor’s degree in Human Services, Healthcare Administration, Social Services, Business Administration, or a related field.

  • Previous experience in administrative support, healthcare, behavioral health, child welfare, social services, human services, case management, referral coordination, or data management.

  • Experience working with databases or case-management systems.

  • Familiarity with DCFS, SACWIS, RTOS, IMSA, or similar systems is a plus.

  • Experience creating, reviewing, or analyzing reports.

Additional Requirements

  • Valid driver’s license.

  • Reliable personal vehicle.

  • Current automobile insurance.

  • Ability to work on-site at both the Chicago and Park Forest locations as business needs require.

  • Ability to work effectively in a fast-paced, deadline-driven environment.

Compensation & Benefits

$18.00–$20.00 per hour, based on experience and qualifications.
