Resident Service Coordinator


Location: Lakeview Apartments - Bethesda, Maryland


Resident Service Coordinator Introduction

Resident Service Coordinators proactively educate residents and connect them with referrals and services, including support for frail and at-risk residents in Affordable/HUD-subsidized housing. The Resident Service Coordinator empowers residents to live independently and exercise their right to self-determination while providing quality federal, state, and community-based referral services that support successful aging in place. This role serves as a liaison between residents, the community, and service providers. Resident Service Coordinators conduct comprehensive non-clinical assessments, connect residents with resources, coordinate services, and provide ongoing follow-up.


I. CORE FUNCTIONS INCLUDE, BUT ARE NOT LIMITED TO:

  • Conduct comprehensive, non-clinical assessments of residents for wellness and social needs.
  • Help residents identify, access, and coordinate services.
  • Encourage resident engagement and participation in service management.
  • Develop educational and preventative health programs.
  • Maintain partnerships with agencies and community providers.
  • Maintain an up-to-date directory of community resources.
  • Demonstrate professionalism and customer service.


II. ADDITIONAL RESPONSIBILITIES:

  • Maintain daily timecards and resident documentation.
  • Build strong partnerships with property management teams.
  • Attend meetings and required training.
  • Navigate benefits and entitlement programs.
  • Adhere to HUD requirements and reporting standards.
  • Travel locally and attend conferences as needed.
  • Maintain a valid driver’s license and reliable transportation.
  • Meet Quality Assurance standards.


III. OTHER RELATED DUTIES

Perform additional duties assigned by management related to service coordination. Report to a Regional Supervisor and adhere to company, HUD, and departmental policies and procedures.


IV. REQUIRED MINIMUM QUALIFICATIONS

Bachelor's degree in a social service-related field (or HUD-approved equivalent experience). Knowledge of elderly and disability services, referral processes, and community resources, with two to three years of social service delivery experience preferred.


V. KNOWLEDGE, SKILLS, AND ABILITIES

Strong verbal and written communication, documentation, critical thinking, problem solving, and professional judgment.


VI. WORK ENVIRONMENT & PHYSICAL DEMANDS

Office and community-based work requiring regular computer use, sitting, standing, walking, and occasional lifting up to 25 pounds. Reasonable accommodations may be made.
