Position Summary
The Wellness Ambassador is a key member of WellSpace Health's Quality team, whose primary role is to improve HEDIS and UDS rates through meaningful patient engagement. This position will work in WellSpace Centers as part of the care team to facilitate addressing care gaps and engaging patients in real time, telephonically, and through care coordination. The Wellness Ambassador will work directly with Quality, clinical, and operational staff across care streams. The Wellness Ambassador will also serve as a subject matter expert for workflows related to Quality measure completion and the continuous quality improvement process.
Required Qualifications
- High school diploma or equivalent required, Bachelor's degree in public health, social work, biology, pre-med, or related field preferred.
- Fluency in additional languages preferred but not required.
- Medical setting knowledge with experience in primary care settings preferred or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
- Experience in community engagement and patient education is required, preferred experience with a Motivational Interviewing approach.
- Demonstrate commitment to providing services for the underserved and working with various people from low-income populations with diverse educational, lifestyle, ethnic, and cultural origins; persons with disabilities, people experiencing homelessness, people experiencing substance dependence, and possessing physical and/or psychological impairment and distress.
- Communicate clearly and concisely, both verbally and in writing and demonstrate a high level of listening skills.
- Demonstrated experience with outreach telephone calls, preferably to a medical patient population.
- Strong customer service experience including active listening, problem solving, and the ability to remain calm in emotional or stressful situations.
- Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
- Compliance with all state and federal laws and regulations as they pertain to position including infection control, sexual harassment, Mandated Reporting, Scope of Practice, OSHA, etc.
- Assist and support the Center/Department/Program to meet standards of High Reliability.
- Demonstrated computing skills, with required proficiency in Microsoft Office Suite, including Teams and Excel, as well as capacity to learn new programs for data reporting and analysis.
Essential Responsibilities
General Clinical Quality and Assurance:
· Support and coordinate data collection and reporting to fulfill the requirements of WellSpace Health’s Quality plan.
· Assist in using Health Maintenance and EHR tools to improve care workflows and patient engagement strategies.
· Assist in evaluating data to identify quality improvement opportunities and enhance clinical outcomes.
· Assist in submitting annual supplemental data to maintain accurate records and reporting to health plans.
Patient and Team Collaboration:
· Collaborate with internal clinical and operational staff, IPAs, and health plans to engage patients in appropriate care, focusing on closing care gaps.
· Participate in huddles and chart scrubbing workflows to identify patients with care gaps or missed care opportunities.
· Schedule patients for primary care, women's health, integrated behavioral health, pediatrics, and other necessary visits, as well as lab appointments.
· Offer patients education on the importance of care gaps and steps taken to close them.
· Act as a member of the Center Care Team, working during visits to intervene and engage patients in their own care and motivate them to complete screenings and attend necessary appointments.
· Register patients for, and explain how to use, the patient portal.
· Serve as a medium between Clinical Quality and the Center environment.
Confidentiality:
· Maintain strict adherence to confidentiality guidelines concerning patient, employee, and organizational communications.
Communication, Care Coordination, and Workflow Execution:
· Utilize organizational expertise to connect patients to various Care Streams to meet their individual needs and overcome barriers to care.
· Attend the weekly Quality meeting, eligible trainings, and other professional development opportunities.
· Offer EHR expertise to Center and Care Team staff, providing solutions to everyday concerns regarding care gap closure and medical record documentation.
· Perform expert, specialized outreach to vulnerable target populations, bringing disengaged patients into WellSpace Health for Initial Health Assessments and care gap closure.
· Other duties as assigned

Physical Demands and Work Environment
The work environment is characteristic of a typical healthcare clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands and arms to reach. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.
The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.
