A nationally recognized, value-based healthcare organization is seeking a Primary Care Nurse Practitioner (NP) to join its collaborative primary care team in Indianapolis, Indiana. Providers are supported by an interdisciplinary care team, including an onsite Health Coach, allowing NPs to focus on delivering comprehensive, whole-person care to patients of all ages rather than high-volume patient visits.

Compensation & Benefits
• Base Salary: $120,000–$160,000+ annually
• Annual Incentive Plan (AIP): Performance-based annual bonus

Schedule
• Full-time outpatient schedule
• 10–15 patients per day
• Appointment lengths ranging from 20–60 minutes
• Shared regional after-hours call rotation
• No routine inpatient responsibilities

Responsibilities
• Provide comprehensive primary care services to patients of all ages, including pediatric, adolescent, adult, and geriatric populations
• Conduct patient histories, physical examinations, well-child checks, and comprehensive health assessments across all age groups
• Diagnose and manage acute and chronic medical conditions within NP scope of practice
• Develop individualized treatment plans based on each patient's clinical needs and family goals
• Prescribe and manage medications as appropriate for pediatric, adult, and geriatric care
• Order, interpret, and follow up on laboratory and diagnostic testing
• Provide preventive care, pediatric immunizations, well-woman/well-child visits, age-appropriate health screenings, and developmental surveillance
• Manage chronic conditions such as asthma, diabetes, hypertension, hyperlipidemia, and other common primary care diagnoses
• Educate patients and families regarding disease prevention, healthy lifestyle choices, and self-management
• Collaborate with physicians, Health Coaches, pediatric specialists, and other members of the interdisciplinary care team
• Utilize onsite laboratory and pharmacy services to streamline patient care
• Participate in population health initiatives and quality improvement programs
• Maintain accurate and timely EMR documentation
• Participate in provider meetings, committee work, and process improvement initiatives
• Support organizational goals related to quality, patient engagement, preventive care, and clinical outcomes
• Build strong, long-term relationships with patients and families through patient-centered care
Requirements
• Master's or Doctoral degree in Nursing (MSN, DNP, or equivalent)
• Current Indiana State Nurse Practitioner license or ability to obtain licensure
• Board certification as a Family Nurse Practitioner (FNP) or other appropriately credentialed primary care NP qualified to treat patients across the lifespan
• National NP certification through an accredited certifying organization (e.g., AANP or ANCC)
• Current Pediatric Advanced Life Support (PALS) and Basic Life Support (BLS) certifications (or ability to obtain PALS)
• Minimum of 3 years of primary care experience treating patients of all ages preferred
• Current DEA registration or ability to obtain
• Negative Tuberculosis (TB) test within 12 months of start date
• Experience working in a team-based care environment
• Experience with electronic medical records; Athena experience preferred
• Experience with coding, documentation, Medicare Advantage, and HCC coding preferred
• Proficiency with Google Workspace preferred
• Strong communication, clinical assessment, and patient/family education skills
• Passion for preventive medicine, family health, population health, and relationship-based care
