Key Responsibilities
Provide comprehensive primary care for older adult patients
Maintain accountability for clinical outcomes and patient experience
Serve as a health coach, guiding lifestyle and long-term health improvements
Coordinate specialty and hospital care while remaining actively involved in patient management
Lead and collaborate with an interdisciplinary care team
Supervise and remain accountable for Advanced Practice Providers (APPs)
Partner with medical and operational leadership to achieve quality and cost goals
Performance Metrics:
Performance in this role is supported and measured by:
Hospital admissions per thousand patients
Appointment utilization (18–21 patient visits per day)
Patient experience scores (CG-CAHPS)
Closure of clinical care gaps
Medical cost management
Qualifications:
MD or DO required
Board Eligible or Board Certified in Internal Medicine, Family Medicine, or Geriatrics
Active or eligible state medical license
Active DEA license
