Job Title: Hospital Liaison – Respiratory & DME Services - Minnesota (Rochester)
Location (On-site, Remote, or Hybrid?): Rochester, MN (onsite)
Contract Duration: PERM

OVERVIEW
This role serves as the critical connection between hospital care and home healthcare services. You'll work closely with hospital teams to ensure patients receive the respiratory equipment and support they need for a safe and successful transition home.

  • Collaborate with hospital case managers, nurses, social workers, physicians, and discharge planners
  • Coordinate timely delivery and setup of respiratory and durable medical equipment
  • Educate patients and caregivers on proper equipment operation, maintenance, and safety
  • Complete insurance verification, referral documentation, delivery tickets, and patient records accurately
  • Support emergency equipment requests and troubleshoot issues as needed
  • Maintain strong relationships with hospital personnel and referral sources
  • Participate in quality improvement initiatives and maintain compliance with company and JCAHO standards
  • Provide in service education and CEU presentations when appropriate
  • Represent Rotech at professional events and community outreach opportunities
  • Travel to patient homes for equipment setup and follow up using a company vehicle

QUALIFICATIONS

  • High school diploma or GED required

Preferred

  • Experience working with medical equipment or in a healthcare environment
  • Medical administration or patient coordination experience
  • Knowledge of insurance verification and medical billing processes
  • Medical terminology
  • Three or more years of related healthcare experience

  • Ability to manage multiple priorities in a fast paced healthcare environment
  • Ability to work independently and collaboratively with hospital teams

TECHNICAL REQUIREMENTS

  • Microsoft Outlook, Word, and Excel
  • Email and electronic documentation systems
  • Internet research and communication
  • Standard office equipment including computers, printers, scanners, and tablets
  • Ability to safely load, unload, transport, and troubleshoot respiratory and DME equipment

PHYSICAL REQUIREMENTS

  • Ability to lift and transport equipment weighing up to 65 pounds
  • Standing, walking, sitting, talking, and listening throughout the workday
  • Close visual work with computer screens and patient documentation
  • Regular contact with patients and medical equipment with potential exposure to contagious pathogens
  • Driving a company vehicle for patient visits and equipment deliveries
