Job Summary

\n

Under limited supervision, leads, plans, and provides educational programs based on the individual needs of the patients and families. This role provides Diabetes Self-Management training (DSMT) and/or Medical Nutrition Therapy (MNT) for individuals with diabetes in both group and individual settings. The role collects, coordinates, and maintains the documentation for Diabetes Self-Management Education Outpatient Program and/or for the Self-Management Education Inpatient Program. The position works with the Clinical Director, Nurse Manager and other educators to maintain ADA/ADCES Recognition status, plan and conduct education programs for team members, patients, the community, dietetic interns and medical students as directed.

  • Leads and provides diabetes education to patients and families including assessing needs of patient, formulating an educational plan, implementing the plan and revision as needed.
  • \n

  • Designs educational activities based on principles of learning, growth and development across the lifespan in conjunction with accepted standards of self-care behavior set forth in the ADCES7 Self Care Behaviors and the Standards of Diabetes Care from the ADA/ADCES. Utilizes a variety of educational approaches including one-on-one instruction, group classes, telemedicine, and audio-visual presentations.

  • Provides information and coordinates patient care with appropriate members of the health care team.
  • \n

  • Participates in the formulation of the goals and objectives for the Diabetes Center and coordinates departmental CQI and/or PI programs.

Work Experience \n
\n

Education & Experience - Required

\n

Registered Dietitian:

\n

    \n
  • Bachelor’s degree in food and nutrition, Dietetics.
  • \n

  • Current licensure in the state of Maryland as a Dietitian/Nutritionist required.
  • \n

  • Four (4) years of clinical experience is required as a Registered Dietitian.
  • \n

  • Must have Specialty Certification as a Certified Diabetes Care and Education Specialist (CDCES).
  • \n

  • Maintenance of 30 CEUs/2 years.
  • \n

OR

\n

\n

Registered Nurse:

\n

    \n
  • Bachelor’s degree in nursing (graduation from an accredited nursing education program approved by the Board of Nursing).
  • \n

  • Current Licensure in the state of Maryland as a Registered Nurse.
  • \n

  • Four (4) years of clinical experience is required as a Registered Nurse.
  • \n

  • Must have Specialty Certification as a Certified Diabetes Care and Education Specialist (CDCES).
  • \n

  • BLS Certification is required.
  • \n

  • Maintenance of 30 CEUs/2 years.
  • \n

\n

Must become a Certified Pump Trainer in at least 1 insulin pump used in the center within one year.
