Salary: $90,000-$106,000 annually (based on experience)

As a Home Dialysis Renal Dietitian, you'll partner with patients, caregivers, and clinical teams to empower healthier, more independent lives—all from the comfort of patients' homes.

Key Responsibilities

Conduct comprehensive nutrition assessments
Develop individualized nutrition care plans
Provide in-home and virtual nutrition counseling
Participate in interdisciplinary care planning
Monitor patient outcomes and laboratory trends
Educate patients and caregivers on renal nutrition
Maintain CMS-compliant documentation
Participate in QAPI and quality improvement initiatives
Support survey readiness and regulatory compliance

Required Qualifications

Bachelor's Degree in Dietetics, Nutrition, or related field.
Registered Dietitian (RD/RDN).
Active Florida Dietitian/Nutritionist License.
Minimum of two (2) years of renal nutrition experience in a dialysis setting, including direct experience supporting Home Hemodialysis (HHD) and/or Peritoneal Dialysis (PD) patients.
Knowledge of CMS ESRD Conditions for Coverage.
Strong communication and patient education skills.
Valid Florida Driver’s License.
Ability to travel throughout assigned territory to patient homes

Preferred Qualifications

Experience supporting established Home Dialysis programs.
Experience working within a CMS-certified ESRD facility or dialysis provider.
Experience with QAPI initiatives.
Survey readiness experience.
Startup healthcare experience.
Board Certified Specialist in Renal Nutrition (CSR).
