Hours: 37.5 hours weekly
Status: Full-time, exempt
Schedule: Monday-Friday, flexible, possibly weekends as needed, on-call
Purpose of Position:
The Quality Assurance and Day Services Manager (QA/DS Manager) is a dynamic and fast-paced role that
provides consistency in the application of systems in the residential division as well as provide oversight,
development, design, and implementation of day supports to people supported in Benhaven’s residential
division. This position is a leader within the residential division. The QA/DS Manager is responsible for
ensuring quality standards and expectations are met, fostering a positive work culture, and nurturing both
staff and the people we support. With an emphasis on team-building, the QA/DS Manager will design and
implement purposeful and meaningful day supports in addition to providing oversight related to the
quality services provided in our residential division. This role requires creativity, insight, ability to
motivate employees, excellent communication, time management, and the ability to work with discretion.
Previous experience with DDS services delivery, HCBS services, and Person-Centered planning is required.
Benhaven is an Employment First agency, for those people supported residentially, if they are interested in
employment those folks will be support through Benhaven’s Career and Transition Services.
Responsibilities:
The following duties are assigned to this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
1. Personnel:
o Serve as a leader on the Residential Leadership Team,
o Oversee and monitor compliance with the Relias Training Platform in collaboration with Human Resources
2. Licensing and Compliance:
o Develop a thorough understanding of DDS CLA and ICF regulations, service definitions, and
documentation requirements.
o Maintain a licensing system for each group home and act as a liaison with DDS regarding elicensing and QSR inspections.
o Submit DDS licensing packets and conduct pre-inspection audits.
o Review e-scores, Fire drills for consistency in application, following DDS requirements, Prompt
managers to complete e-scores as needed, submit e-scores to DDS
o Coordinate and submit plans of correction
o Participate in home inspections and review T-Logs to provide feedback.
3. Quality Assurance:
o Complete quarterly audits for each home focusing on key areas (Health & Safety, Personal
Achievement, Physical Plant, Staffing).
o Periodic reviews of Therap and ensure compliance with DDS policies
o In collaboration with the Residential Division Director, ensures consistency across all Benhaven
CLA homes.
o Audit day services
o Point person for all residential referrals and coordinate review team
o GER review and submission to DDS – escalating GER’s as needed
o Ensure Critical incident process is followed
4. Day Services
o Provide strategic direction to all people receiving day services through Benhaven in our
residential division,
o Ensure our day services are person center,
o People receiving day services are leading meaningful and purposeful lives through values and
preferred activities,
o Our employment and day services are meeting with requirements authorized by DDS,
o Develop day staff – hiring, developing schedules, training etc.,
o Ensure there is a clear understanding and division between day services and residential
services,
o Plan and implement recommendations made in the day services analysis,
o Work closely with Residential Managers to implement days services and structures,
o Continuously work on staying on top of cutting-edge services in day services,
o Attend regular meetings with the CEO and Residential Division Director.
Requirements:
• Proven experience in residential and employment/day services
• Has a strong understanding of DDS regulations, standards, policy, and practices.
• Strong organizational skills and the ability to multitask and prioritize.
• Excellent communication skills, both written and verbal.
• Experience with Relias Training Platform and other relevant software is preferred.
• Ability to work collaboratively within a team.
• Strong leadership abilities with experience in managing personnel and compliance.
