Caregiver Job Description (Home Health & Hospice)


Position Summary


The Caregiver provides essential non-medical and personal care services to patients in their homes or hospice settings under the supervision of a Registered Nurse (RN) or clinical care team. The caregiver supports patients with activities of daily living (ADLs), promotes comfort, safety, dignity, and emotional well-being, and ensures quality care in accordance with the patient care plan.

Key Responsibilities


  • Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility
  • Provide personal care and hygiene support in a safe and respectful manner
  • Assist with feeding and meal preparation as needed
  • Monitor and report changes in patient condition to the supervising nurse
  • Provide companionship and emotional support to patients and families
  • Assist with light housekeeping tasks (laundry, cleaning patient area, changing bed linens)
  • Support patients with safe transfers and mobility assistance (walker, wheelchair, transfer belt)
  • Follow care plan instructions developed by the RN or care team
  • Maintain patient confidentiality (HIPAA compliance) at all times
  • Document care activities as required by agency policy
  • Follow infection control and safety procedures (OSHA standards)
  • Communicate effectively with RN, case manager, and interdisciplinary team

Required Qualifications / Skills


  • Compassionate, patient, and caring attitude toward elderly and disabled patients
  • Ability to follow written and verbal instructions
  • Good communication and interpersonal skills
  • Physical ability to lift, move, or assist patients as required
  • Reliable transportation (if required for home visits)
  • Ability to work independently in patient homes
  • Understanding of basic infection control and safety practices

Education Requirements


Minimum Requirements


  • High School Diploma or GED (required)
