Contract Duration: 26 Weeks
Shift: Evening Shift, 3:30 PM – 12:00 AM
Schedule: 5 × 8-hour shifts per week

Total Weekly Gross: $1,821

Position Summary

We are seeking a qualified Licensed Practical Nurse (LPN) to provide compassionate, resident-centered care in a long-term care setting. The selected candidate will support residents with daily care needs, administer medications, monitor health conditions, and collaborate with registered nurses and certified nursing assistants to maintain residents’ safety and well-being.

Key Responsibilities

• Provide nursing care to approximately 20–40 residents under the supervision of a Registered Nurse (RN).
• Administer medications and perform wound care and skin assessments in accordance with established protocols.
• Monitor residents’ health conditions and promptly report changes to the RN or physician.
• Supervise and guide Certified Nursing Assistants (CNAs) in providing resident care and assisting with daily activities.
• Maintain accurate and timely clinical documentation.
• Participate in shift handoffs and contribute to resident care planning.
• Assist residents with activities of daily living (ADLs), mobility, and safety needs.
• Promote residents’ emotional well-being and provide respectful, compassionate care.

Requirements

• Active and valid New York State LPN license.
• Relevant experience in long-term care (LTC) or skilled nursing facilities.
• Current Basic Life Support (BLS) certification.
• Ability to provide resident-centered care and supervise CNAs effectively.
• Compliance with the facility’s vaccination requirements; medical exemptions may be considered.
