We are seeking a compassionate and detail-oriented Patient Care Coordinator to support&nbsp;administrative functions within our healthcare practice. This role serves as a key point of contact for patients, ensuring a smooth, efficient, and positive care experience from check-in through follow-up. The ideal candidate is patient-focused, highly organized, and comfortable balancing hands-on clinical duties with care coordination responsibilities.
This position will primarily be based at our Tarzana office.
Key Responsibilities

<li>Serve as the primary point of contact for patients, coordinating appointments, referrals, and follow-up care</li>
<li>Support patient intake, scheduling, and documentation within the EHR system</li>
<li>Educate patients on procedures, care plans, and next steps to ensure understanding and compliance</li>
<li>Coordinate diagnostic testing, lab work, and external referrals</li>
<li>Maintain accurate and timely patient records in compliance with HIPAA and clinical standards</li>
<li>Assist with prior authorizations, insurance verification, and patient communication as needed</li>
<li>Collaborate closely with providers, nurses, and administrative staff to optimize patient flow</li>

Qualifications

<li>Certified Medical Assistant&nbsp;</li>
<li>2+ years of experience in a clinical healthcare setting preferred</li>
<li>Strong patient communication and interpersonal skills</li>
<li>Proficiency with EHR systems and basic medical documentation</li>
<li>Ability to multitask and stay organized in a fast-paced environment</li>
<li>Knowledge of HIPAA, infection control, and clinical safety standards</li>
